摘　　要：目的 分析糖尿病周围神经病变治疗中应用硫辛酸联合胰岛素强化方案的价值。方法 选取2023年3月—2024年6月上海市嘉定区迎园医院收治的糖尿病周围神经病变患者102例,按不同治疗方案分组,51例/组,对照组用硫辛酸+二甲双胍治疗、观察组用硫辛酸+胰岛素强化治疗。比较两组神经电生理指标、血糖水平、临床疗效、不良反应发生率。结果 与对照组比较,观察组治疗总有效率更高,不良反应发生率更低,差异均有统计学意义(P均<0.05)。治疗后,与对照组相比,观察组正中神经、腓总神经、尺神经运动与感觉传导速度均更高,空腹血糖、糖化血红蛋白水平均更低,差异均有统计学意义(P均<0.05)。结论 硫辛酸联合胰岛素强化方案可改善糖尿病周围神经病变的神经电生理指标及血糖水平,确保治疗有效性及安全性。Objective To analyze the value of lipoic acid combined with insulin intensive regimen in the treatment of diabetic peripheral neuropathy.Methods From March 2023 to June 2024,102 patients with diabetic peripheral neuropathy admitted to Yingyuan Hospital of Jiading District in Shanghai were selected.Grouped according to different treatment regimens,51 cases/group,the control group was treated with lipoic acid + metformin,the observation group was treated with lipoic acid + insulin intensive therapy.The clinical efficacy,neuroelectrophysiological indexes,blood glucose level and incidence of adverse reactions were compared between groups.Results Compared with the control group,the total effective rate of the observation group was higher and the incidence of adverse reactions was lower,the differences were statistically significant(both P<0.05).After treatment,compared with the control group,the motor and sensory conduction velocities of median nerve,common peroneal nerve and ulnar nerve in the observation group were higher,the levels of fasting plasma glucose and glycated hemoglobin A1c were lower,the differences were statistically significant(all P<0.05).Conclusion Lipoic acid combined with insulin intensive regimen can improve the neurophysiological indexes and blood glucose levels of diabetic peripheral neuropathy,and ensure the efficacy and safety of treatment.
