摘 要:目的探究糖化血红蛋白与空腹血糖在糖尿病检测中的意义。方法择取2023年1—12月福建省龙岩市第一医院收治的110例疑似糖尿病患者,所有患者均接受空腹血糖、糖化血红蛋白检测,以临床综合诊断为金标准,对比分析单一检测与联合检测的诊断效果;将临床综合诊断确诊糖尿病患者为阳性组,未确诊为阴性组,对比分析两组血糖水平情况。结果临床综合诊断确诊阳性105例;空腹血糖单一检测检出真阳性86例,糖化血红蛋白单一检测检出真阳性95例;联合检测检出真阳性100例。联合检测的灵敏度、准确度高于空腹血糖、糖化血红蛋白单一检测,漏诊率低于空腹血糖、糖化血红蛋白单一检测,差异均有统计学意义(P均<0.05)。阳性组糖化血红蛋白为(8.33±1.08)%,空腹血糖为(10.86±0.89)mmol/L,均较阴性组的(5.21±0.71)%、(7.12±0.37)mmol/L更高,差异均有统计学意义(t=6.379、9.324,P均<0.001)。结论糖化血红蛋白与空腹血糖联合检测的诊断效果较高,在临床诊断中具有一定的应用价值。Objective To explore the detection significance of glycated hemoglobin A1c and fasting plasma glucose in diabetes mellitus.Methods A total of 110 patients with suspected diabetes mellitus admitted to Fujian Longyan First Hospital from January to December 2023 were selected.All patients received fasting plasma glucose and glycated he-moglobin A1c tests.The clinical comprehensive diagnosis was used as the gold standard,and the diagnostic effects of single detection and combined detection were compared and analyzed.The patients with diabetes mellitus diagnosed by clinical comprehensive diagnosis were taken as the positive group,and the patients were not diagnosed as the nega-tive group.The blood glucose levels of the two groups were compared and analyzed.Results 105 cases were diagnosed as positive by clinical comprehensive diagnosis.There were 86 true positive cases by fasting plasma glucose test and 95 true positive cases by glycated hemoglobin A1c test.The combined test detected 100 true positive cases.The sensi-tivity and accuracy of combined detection were higher than those of single detection of fasting plasma glucose and gly-cated hemoglobin A1c,and the rate of missed diagnosis was lower than that of single detection of fasting plasma glu-cose and glycated hemoglobin A1c,and the differences were statistically significant(all P<0.05).The positive group had higher glycated hemoglobin A1c as(8.33±1.08)%and fasting plasma glucose as(10.86±0.89)mmol/L than(5.21±0.71)%and(7.12±0.37)mmol/L in the negative group,and the differences were statistically significant(t=6.379,9.324,both P<0.001).Conclusion The combined detection of glycated hemoglobin A1c and fasting plasma glucose has a high diagnostic effect and has certain application value in clinical diagnosis.
