甲钴胺穴位注射治疗2型糖尿病周围神经病变的临床效果分析

摘　　要：目的分析2型糖尿病周围神经病变患者应用甲钴胺穴位注射治疗的效果。方法选取2022年7月—2024年6月就诊于陕西糖尿病医院中医内分泌科的60例2型糖尿病周围神经病变患者,按照不同治疗方法分为两组,各30例。对照组予硫辛酸静脉滴注治疗,研究组予硫辛酸+甲钴胺穴位注射治疗。比较两组血糖水平、胰岛功能、神经传导速度、治疗效果和不良反应发生率。结果研究组治疗效果更高,差异有统计学意义(P<0.05)。治疗后,相比于对照组,研究组空腹及餐后2 h血糖、空腹胰岛素水平、胰岛素抵抗指数均更低,胰岛β细胞功能指数、正中运动神经传导速度、正中感觉神经传导速度、腓总感觉神经传导速度均更高,差异均有统计学意义(P均<0.05)。研究组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论对2型糖尿病周围神经病变患者行甲钴胺穴位注射治疗,可改善患者症状,调节血糖水平;同时也能改善胰岛功能,加快神经传导速度。Objective To analyze the effect of acupoint injection of mecobalamin in patients with type 2 diabetic pe-ripheral neuropathy.Methods Sixty patients with type 2 diabetic peripheral neuropathy from the Department of Endo-crinology of Traditional Chinese Medicine in Shaanxi Diabetes Hospital from July 2022 to June 2024 were selected and divided into two groups according to different treatment methods,with 30 cases in each group.The control group was treated with intravenous infusion of lipoic acid,and the study group was treated with acupoint injection of lipoic acid+mecobalamin.The blood glucose level,islet function,nerve conduction velocity,therapeutic effect and adverse reactions were compared between the two groups.Results The therapeutic effect in the study group was higher,and the difference was statistically significant(P<0.05).After treatment,compared with the control group,the levels of fast-ing and 2-hour postprandial blood glucose,fasting insulin and insulin resistance index in the study group were lower,the isletβcell function index,median motor nerve conduction velocity,median sensory nerve conduction velocity and peroneal common sensory nerve conduction velocity were higher,the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the study group and control group(P>0.05).Conclusion Acupoint injection of mecobalamin in patients with type 2 diabetic peripheral neuropathy can improve the symptoms of patients and regulate blood glucose levels,at the same time,it can also improve islet function and accelerate nerve conduction velocity.
