体外冲击波联合高压氧疗法治疗糖尿病足慢性溃疡的应用研究

摘　　要：目的研究体外冲击波联合高压氧疗法治疗糖尿病足慢性溃疡的应用效果。方法选择2022年3月—2024年8月湘潭市第六人民医院收治的60例糖尿病足慢性溃疡患者,依照不同的治疗方法分为参考组(30例,采取体外冲击波治疗)、观察组(30例,采取体外冲击波联合高压氧疗法治疗)。比较两组血流速度、炎性因子水平、溃疡面积缩小面积、创面愈合时间、并发症发生率。结果治疗后,观察组腘动脉血流速度、足背动脉血流速度均高于参考组,白细胞介素-6、C反应蛋白水平均低于参考组,溃疡面积缩小面积、创面愈合时间均优于参考组,差异均有统计学意义(P均<0.05)。观察组并发症发生率略低于参考组,但差异无统计学意义(P>0.05)。结论采取体外冲击波联合高压氧疗法治疗,可以使糖尿病足慢性溃疡患者的血流速度加快,同时还可减轻炎症反应,缩小溃疡面积,加快创面愈合。Objective To study the application effect of extracorporeal shock wave combined with hyperbaric oxygen therapy in the treatment of diabetic foot chronic ulcer.Methods From March 2022 to August 2024,60 patients with diabetic foot chronic ulcer admitted to the Sixth People's Hospital of Xiangtan were selected and divided into refer-ence group(30 cases,treated with extracorporeal shock wave therapy)and observation group(30 cases,treated with ex-tracorporeal shock wave combined with hyperbaric oxygen therapy)according to different treatment methods.The blood flow velocity,inflammatory factor levels,ulcer area reduction area,wound healing time and incidence of compli-cation were compared between the two groups.Results After treatment,the popliteal artery blood flow velocity and dorsalis pedis artery blood flow velocity of the observation group were higher than those of the reference group,the lev-els of interleukin-6 and C-reactive protein were lower than those of the reference group,the ulcer area reduction area and wound healing time were better than those of the reference group,and the differences were statistically significant(all P<0.05).The incidence of complications in the observation group was slightly lower than that in the reference group,but the difference was not statistically significant(P>0.05).Conclusion Extracorporeal shock wave combined with hyperbaric oxygen therapy can accelerate the blood flow velocity of patients with diabetic foot chronic ulcer,re-duce the inflammatory reaction,reduce the ulcer area and accelerate the wound healing.
