钙通道阻滞剂联合叶酸对老年H型高血压患者血压控制效果研究

摘　　要：目的探讨钙通道阻滞剂(CCB)与叶酸联合应用对老年H型高血压患者的血压控制效果。方法选取2022年03月-2024年04月我院收治的98例老年H型高血压患者,采用随机信封法将患者随机分为叶酸组(n=49)和联合组(n=49),叶酸组给予马来酸依那普利片和叶酸片,联合组在叶酸组基础上给予硝苯地平缓释片(Ⅱ),比较治疗前、治疗1个月和治疗3个月的血压[收缩压(SBP)、舒张压(DBP)]水平变化;比较治疗前后叶酸组和联合组受试者血脂指标[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(LDL-C)、低密度脂蛋白(HDL-C)]和炎症因子[同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)]水平变化;统计治疗过程中水肿、心悸、头痛、眩晕、皮疹等不良反应的发生情况。结果治疗后1个月和3个月SBP和DBP均较治疗前降低(P<0.05),治疗3个月SBP和DBP较治疗1个月降低(P<0.05),联合组治疗1个月和治疗3个月时的SBP和DBP均显著低于叶酸组(P<0.05);治疗后两组TC、TG和LDL-C水平均低于治疗前(P<0.05),HDL-C水平高于治疗前(P<0.05),联合组TC、TG、LDL-C和HDL-C水平与叶酸组无差异(P>0.05);两组治疗后Hcy、hs-CRP、TNF-α水平均低于治疗前(P<0.05),联合组Hcy、hs-CRP、TNF-α水平低于叶酸组,差异无统计学意义(P>0.05);治疗过程中叶酸组和联合组均无不良反应发生。结论CCB联合叶酸对老年H型高血压患者具有良好的临床疗效,能够显著降低血压,且具有一定调节血脂和降低炎症因子水平的疗效,安全性较好。Objective To investigate the effect of calcium channel blocker(CCB)combined with folic acid on blood pressure control in elderly patients with H type hypertension.Methods 98 elderly patients with H-type hypertension admitted to our hospital from March 2022 to April 2024 were randomly divided into folic acid group(n=49)and combined group(n=49)by random envelope method.Folic acid group was given enalapril maleate tablet and folic acid tablet,and combined group was given nifedipine sustained-release tablet on the basis of folic acid group(Ⅱ).And the blood pressure(SBP)and diastolic blood pressure(DBP)levels were compared before treatment,1 month and 3 months after treatment.The levels of serum lipids(total cholesterol(TC),triglycerides(TG),high density lipoprotein(LDL-C),low density lipoprotein(HDL-C))and inflammatory factors(homocysteine(Hcy),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α))were compared before and after treatment.The incidence of adverse reactions such as edema,palpitation,headache,vertigo and rash during treatment were analyzed.Results SBP and DBP were significantly lower at 1 month and 3 months after treatment than before treatment(P<0.05),and SBP and DBP at 3 months after treatment were significantly lower than those in folic acid group at 1 month and 3 months(P<0.05).The levels of TC,TG and LDL-C were lower after treatment than before treatment(P<0.05),and the levels of HDL-C were higher than before treatment(P<0.05).There was no difference between the levels of TC,TG,LDL-C and HDL-C in combination group and folic acid group(P>0.05).The levels of Hcy,hs-CRP and TNF-αin 2 groups after treatment were lower than before treatment(P<0.05),and the levels of Hcy,hs-CRP and TNF-αin combination group were lower than those in folic acid group,with no statistical significance(P>0.05).No adverse reactions occurred in the folic acid group and the combined group.Conclusion:CCB combined with folic acid has a good clinical effect on elderly H-type hypertension patients,which can
