活血化瘀类中药注射剂的使用分析

摘　　要：目的探讨医院活血化瘀类中药注射剂的临床应用现状及安全性评估。方法利用医院HIS系统抽取2019~2022年使用活血化瘀类中药注射剂的住院患者13005例(涉及医嘱15401份),采用回顾性分析法,统计病历资料中的用药情况及不良反应发生率。结果红花黄色素氯化钠注射液、注射用红花黄色素、注射用丹参多酚酸盐、银杏内酯注射液、注射用血塞通、苦碟子注射液、舒血宁注射液、注射用血栓通、冠心宁注射液为医院常用活血化瘀类中药注射剂;15401份医嘱中超疗程用药占23.83%,溶媒使用不当占14.54%,超剂量用药占0.94%,超说明书用药占34.66%,超使用禁忌用药占0.01%;在接受活血化瘀类中药注射剂治疗的13005例患者中,出现药品不良反应的病例共计290例,不良反应发生率为2.23%,其中有133例患者用药不合理,占比为45.86%。结论临床活血化瘀类中药注射剂存在不合理使用情况,应加强重点监控,保证用药安全。同时,不良反应的发生亦与药品本身、患者体质等因素有关。Objective To explore the status clinical application and safety assessment of traditional Chinese medicine(TCM)injections for promoting blood circulation and removing blood stasis.Methods The data of TCM injections for promoting blood circulation and removing blood stasis on drug utilization of the medical records and the incidence of adverse reactions of 13005 hospitalized patients(15401 medical orders)by retrospective analysis were used by HIS system extraction in 2019~2022.Results Safflower yellow pigment sodium chloride injection,safflower yellow pigment for injection,salvia polyphenolate for injection,Ginkgolide injection,Xuesaitong injection,Kudiezi injection,Shuxuening injection,Xueshuantong injection and Guanxinning injection were commonly TCM injections for promoting blood circulation and removing blood stasis in the hospital.Among the 15401 medical orders,the proportion of over-course drug use was 23.83%,the proportion of improper use of solvent was 14.54%,the proportion of over-dose was 0.94%,the proportion of off-label drug use was 34.66%,and the proportion of over-contraindicationuse was 0.01%.In 13005 cases of TCM injections for promoting blood circulation and removing blood stasis,adverse reactions of a total of 290 cases were found,among which 133 cases were irrational drug use,accounting for 45.86%.Conclusion The use of TCM injections for promoting blood circulation and removing blood stasis exist unreasonable status,so the safety of medication should be strengthened monitoring.The occurrence of adverse reactions is related to the medication,the patients'constitution and other factors.
