摘　　要：目的探究医院—社区—家庭联动血糖管理的应用效果。方法选取某医院2023年7—12月就诊的糖尿病患者72例作为研究对象,随机分为对照组和干预组。对照组患者给予常规干预,干预组给予医院—社区—家庭联动血糖管理,比较干预前后两组患者的血糖水平、自我管理依从性和健康管理行为水平。结果干预后,干预组的血糖水平和健康管理行为改善情况均优于对照组。倍差法检验结果显示干预净效应有统计学意义(P<0.05),并且干预组的自我管理依从性高于对照组(P<0.05)。结论选用医院—社区—家庭联动血糖管理对糖尿病患者进行干预,对于加强患者的血糖控制能力、自我管理依从性具有较高价值。Objective To explore the application effect of hospital-community-family linkage blood glucose management.Methods A total of 72 cases of diabetic patients in a hospital from July to December 2023 were selected as the research object and randomly divided into control group and intervention group.The control group was given routine intervention,and the intervention group was given hospital-community-family linkage blood glucose management.The blood glucose level,selfmanagement compliance and health management behavior level of the two groups were compared after intervention.Results After the intervention,the blood glucose levels and the improvements of health management behavior in the intervention group were better than those in the control group,and the Difference-in-Difference test showed that the net effect of the intervention was statistically significant(P<0.05),and the self-management compliance of the intervention group was higher than that of the control group(P<0.05).Conclusion The use of hospital community family linkage blood glucose management continuation program for the intervention of patients with diabetes had high value for strengthening the blood glucose control ability and self-management compliance of patients.
