摘 要:目的探讨神经干细胞(NSCs)经不同途径移植定向精准修复创伤性脑损伤(TBI)大鼠的疗效。方法使用随机数字表法将75只雄性大鼠分为假手术对照组、模型对照组、患处NSCs移植组、鼻内NSCs移植组、尾静脉NSCs移植组,采用改良式自由落体法制备中度TBI模型,分别以局部、经鼻内、尾静脉注射等途径给予大鼠NSCs。术后第1日及第7日通过改良神经功能缺损评分(mNSS)考察大鼠术后神经功能恢复情况,解剖观察脑组织术后恢复情况,免疫组化比较活化的促凋亡因子(Cleaved-Caspase3)、增殖细胞核抗原(PCNA)、SUMO特异性肽酶3(SENP3)阳性细胞数。结果(1)改良神经功能缺损评分(mNSS)结果显示,与假手术对照组相比,模型对照组第1日及第7日mNSS评分均明显升高,差异有统计学意义(P<0.01);与模型对照组比较,患处NSCs移植组、鼻内NSCs移植组、尾静脉NSCs移植组神经功能评分均降低,差异有统计学意义(P<0.01)。(2)解剖观察,与模型对照组比较,术后第7日各NSCs移植组大鼠脑损伤均有明显改善。(3)术后第7日与假手术对照组比较,模型对照组大鼠的脑组织湿重、脑脏器系数均升高,差异有统计学意义(P<0.05);与模型对照组比较,患处NSCs移植组脑组织湿重、脑脏器系数均降低,差异有统计学意义(P<0.01~0.05)。(4)免疫组化结果提示:与模型对照组比较,各NSCs移植组中Cleaved-Caspase3、PCNA、SENP3阳性细胞数均显著降低,差异有统计学意义(P<0.05)。与尾静脉NSCs移植组相比,患处NSCs移植组和鼻内NSCs移植组的Cleaved-Caspase3、PCNA及SENP3阳性细胞数降低更显著(P<0.01)。结论神经干细胞移植可有效促进TBI大鼠的神经功能恢复,其中经鼻内途径与患处NSCs移植途径治疗效果优于尾静脉途径。Objective To explore the efficacy of neural stem cells(NSCs)transplanted via different routes for targeted and precise repair of traumatic brain injury(TBI)in rats.Methods 75 male rats were divided into a sham-operated control group,a model control group,an affected area NSCs transplantation group,an intranasal NSCs transplantation group and a tail vein NSCs transplantation group by using the modified free-fall method,and NSCs were given to the rats by local,intranasal and tail vein injections,etc.The neural function recovery of the rats was examined by modified neurological deficit scores(mNSS)on the 1st and 7th postoperative days,and the postoperative recovery of brain tissues was observed by autopsy,and the number of Cleaved-Caspase3,PCNA(proliferating cell nuclear antigen)and SENP3-positive cells were compared by immunohistochemistry.Results(1)The results of modified neurological deficit score(mNSS)scores showed that compared with the sham-operated control group,the scores of the model control group on day 1 and day 7 were significantly higher,with statistically significant differences(P<0.01);compared with the model control group,the neurological scores of the affected area NSCs transplantation group,the intranasal NSCs transplantation group,and the tail vein NSCs transplantation group were all lower,with statistically significant differences(P<0.01).(2)Anatomical observation,compared with the model control group,there was a significant improvement in the brain injury of the rats in each NSCs transplantation group on the 7th postoperative day.(3)Comparing with sham-operated control group on the 7th postoperative day,the wet brain weight and brain organ coefficient of the rats in the model control group were increased,and the difference was statistically significant(P<0.05);comparing with the model control group,the wet brain weight and brain organ coefficient of the affected area NSCs transplantation group was decreased,and the difference was statistically significant(P<0.01~0.05).(4)Immunohistochemistry res
