基于“肺与大肠相表里”理论从血瘀角度探析溃疡性结肠炎

摘　　要：基于“肺与大肠相表里”理论,从血瘀角度探析溃疡性结肠炎的中医病因病机、现代医学发病机制以及辨治思路。认为肺与大肠经络相通、表里同气、气血相关;从血瘀角度提出,本病病因病机为肺气虚损、气弱血留,肺气闭阻、气滞血停,肺热移肠、灼络血出,肠病遗肺、肺肠通应;结合现代研究,从免疫炎症、微生物菌群失调和氧化应激异常佐证了“肺与大肠”在本病发病机制中的作用;治疗上当益气化瘀、行气散瘀、清气逐瘀。Based on the"interior-exterior relationship between the lung and large intestine"theory,this paper,with a focus on blood stasis,explores the traditional Chinese medicine(TCM)etiology,pathogenesis and differentiation and treatment strategies of ulcerative colitis(UC),integrating modern medical insights into disease mechanism.This paper holds that the lung and the large intestine are connected through meridians,share the same qi as an exterior-interior relationship,and are interrelated in qi and blood.From the perspective of blood stasis,the etiology and pathogenesis of UC are proposed to involve a deficiency of lung qi leading to weak qi and blood retention,lung qi obstruction causing both qi stagnation and blood stasis,lung heat transferring to the intestines and causing blood extravasation,and intestinal disorders affecting the lung as they are interconnected in TCM.Integrating modern research,the roles of the"lung and large intestine"in the pathogenesis of UC are supported by evidence from immune inflammation,dysbiosis of the microbiota,and oxidative stress abnormalities.Treatment strategies should focus on benefiting qi to resolve blood stasis,promoting qi circulation to dissipate blood stasis,and clearing heat to expel stasis.
