清热法治疗溃疡性结肠炎研究进展

摘　　要：溃疡性结肠炎(UC)是临床常见的消化系统疑难病,清热法是中医药治疗UC的核心法则。通过梳理文献,综述UC不同发展阶段的清热法代表方药。湿热初起,病理因素单一,可选用以黄连为代表的清热燥湿方药;热毒炽盛,可选用清热解毒方药,代表药物为白头翁;血分热盛,则当用清热凉血药,如地榆类方;瘀热肉腐,则须在清热解毒、凉血的基础上加用具有祛瘀排脓功效的药物,如败酱类方。此外,UC病程中还可选用青黛类方药清肝火,进而改善“土虚木乘”的病理状态。Ulcerative colitis(UC)is a common and intractable digestive system disease in clinical practice.The heat-clearing method is central to traditional Chinese medicine(TCM)treatment for UC.This article summarizes the representative of heat-clearing method formulas for different stages of UC by reviewing the literatures.At the intial stage of damp-heat,where the pathological factors are simple,heat-clearing and damp-drying herbal formulas can be selected,with Huanglian(Coptidis Rhizoma)as the representative herb.For blazing heat-toxin syndrome,heat-clearing and toxin-removing herbal formulas can be selected,with Baitouweng(Pulsatillae Radix)as the representative herb.When there is excessive heat in the blood aspect,heat-clearing and blood-cooling formulas should be used,such as formulas containing Diyu(Sanguisorbae Radix).When there is static heat causing flesh decay,it is necessary to use Chinese herbal medicines with the effect of removing blood stasis and expelling pus on the basis of heat-clearing,toxin-removing,and blood-cooling treatments,such as formulas containing Baijiang(Parinia Uillosa).Additionally,during the course of UC,formulas containing Qingdai(Indigo Naturalis)can be selected to clear liver fire,thereby improving the pathological state of"earth deficiency leading to wood overacting".
