摘 要:目的:研究多种油脂肪乳(SMOF)营养支持于脑损伤早产儿神经功能康复中的价值。方法:选取2020年10月—2024年8月收治的脑损伤早产儿100例,采用随机数字表法分为两组。两组均给予对症治疗,常规组给予常规营养支持干预,SMOF组给予SMOF营养支持干预。记录两组初排胎便时间、胎便转黄时间、体重下降最大幅度、恢复出生体重时间、脂肪乳/氨基酸用量达3.0g/(kg·d)日龄、总热卡达110 kcal/(kg·d)日龄以及喂养不耐受、贫血、脑室出血、脑积水、脑室增大等并发症发生率。检测两组神经元特异性烯醇化酶(NSE)、髓磷脂碱性蛋白(MBP)、S100β、白蛋白(ALB)、视黄醇结合蛋白(RBP)、前白蛋白(PA)水平。结果:SMOF组初排胎便时间(18.16±3.77)h、胎便转黄时间(42.56±5.07)h、恢复出生体重时间(9.21±1.55)d短于常规组[(19.81±4.05)h、(44.97±6.16)h、(9.94±1.74)d],脂肪乳用量达3.0g/(kg·d)日龄(5.98±2.36)d、氨基酸用量达3.0g/(kg·d)日龄(3.62±1.25)d、总热卡达110kcal/(kg·d)日龄(11.61±2.36)d、体重下降最大幅度(5.76±1.20)%与常规组[(6.14±2.23)d、(3.67±1.14)d、(11.74±2.25)d、(5.89±1.14)%]比较,差异无统计学意义(P>0.05)。治疗前,两组血清相关因子比较差异无统计学意义(P>0.05);两组治疗后NSE、MBP、S100β均较治疗前降低,且SMOF组治疗后NSE、MBP、S100β低于常规组(P<0.05);两组治疗后ALB、RBP、PA均较治疗前升高,且SMOF组治疗后ALB、RBP、PA高于常规组(P<0.05)。SMOF组喂养不耐受、贫血、脑室出血、脑积水、脑室增大等并发症发生率比较,差异无统计学意义(P>0.05)。结论:SMOF营养支持可改善脑损伤早产儿营养状态,抑制NSE、MBP、S100β的表达,促进疾病康复。【Objective】To investigate the value of nutritional support with multiple oil-fat milk(SMOF)in neurological rehabilitation of brain-injured preterm infants.【Method】Totally 100 cases of preterm infants with brain injury admitted from October 2020 to August 2024 were selected and divided into two groups using random number table method.Both groups were given symptomatic treatment,conventional group was given conventional nutritional support intervention and SMOF group was given SMOF nutritional support intervention.The time of first evacuation of fetal stool,the time of fetal stool turning yellow,the maximum weight loss,the time of returning to birth weight,the fat milk/amino acid dosage up to 3.0 g(/kg·d)per day of age,the total calorific value up to 110 kcal(/kg·d)per day of age,and the incidence of feeding intolerance,anaemia,ventricular haemorrhage,hydrocephalus,enlarged ventricles,and other complications in the two groups were recorded.Neuron-specific enolase(NSE),myelin basic protein(MBP),S100β,albumin(ALB),retinol-binding protein(RBP),and prealbumin(PA)levels were detected in both groups.【Result】In the SMOF group,the time to first defecation(18.16±3.77)h,the time to yellowing of fetal stool(42.56±5.07)h,the time to recovery of birth weight(9.21±1.55)d were shorter than those in the conventional group[(19.81±4.05)h,(44.97±6.16)h,(9.94±1.74)d],the amount of fat milk up to 3.0 g(/kg·d)d(5.98±2.36)d,amino acid dosage up to 3.0 g(/kg·d)d(3.62±1.25)d,total calories up to 110 kcal(/kg·d)d(11.61±2.36)d,and the maximum body weight loss of(5.76±1.20)%compared to the conventional group[(6.14±2.23)d,(3.67±1.14)d,(11.74±2.25)d,(5.89±1.14)%],the difference was not statistically significant(P>0.05).Before treatment,there was no statistically significant difference in the comparison of serum-related factors between the two groups(P>0.05).NSE,MBP,and S100βwere lower in both groups after treatment compared with before treatment,and NSE,MBP,and S100βwere lower in the SMOF group than in the conv
