摘  要:目的:全面调查慢性心衰患者营养不良的发生风险,并对患者实施个体化营养支持,分析改善效果。方法:研究选择我院2023年1月—2024年1月接收的慢性心衰患者106例为对象,均实施营养危险指数(NRI)、控制营养状况(CONUT)调查,以调查结果为依据,分为营养不良组与营养正常组,比较两组的基本信息情况。采用Logistic多因素回归分析影响慢性心衰患者营养不良风险的因素,并对营养不良组实施个体化营养支持,比较干预前、后的效果。结果:营养不良组占比为63.21%(67/106);营养正常组占比36.79%(39/106)。两组的年龄、体质量指数、血清白蛋白(ALB)、淋巴细胞计数、总胆固醇(TC)之间的差异存在统计学意义(P<0.05)。Logistic多因素回归显示,年龄、体质量指数、ALB、淋巴细胞计数、TC是慢性心衰患者营养不良的重要风险因子。实施个体化营养支持后,营养不良组的ALB、淋巴细胞计数、TC水平均较干预前更高(P<0.05)。结论:年龄、体质量指数、ALB、淋巴细胞计数、TC是影响慢性心衰患者营养不良风险的独立危险因子,临床应引起足够的重视,同时,采用个体化营养支持有利于改善慢性心衰患者的营养状态,从而促进预后水平的提高。【Objective】To comprehensively investigate the risk of malnutrition in patients with chronic heart failure,provide personalized nutritional support to patients,and analyze the improvement effect.【Method】A total of 106 patients with chronic heart failure admitted to our hospital from January 2023 to January 2024 were selected as the study subjects.Nutritional Risk Index(NRI)and Controlled Nutritional Status(CONUT)surveys were conducted on them.Based on the survey results,they were divided into malnutrition group and normal nutrition group.The basic information of the two groups was compared,and Logistic Multiple Regression Analysis was used to identify the factors that affect the risk of malnutrition in patients with chronic heart failure.Personalized nutritional support to the malnutrition group was provided,and the effects before and after intervention were compared.【Result】The proportion of malnutrition group was 63.21%(67/106).The group with normal nutrition accounted for 36.79%(39/106).There were statistically significant differences in age,body mass index,serum albumin(ALB),lymphocyte count,and total cholesterol(TC)between the two groups(P<0.05).Logistic multiple regression analysis showed that age,body mass index,ALB,lymphocyte count,and TC are important risk factors for malnutrition in patients with chronic heart failure.After implementing personalized nutritional support,the ALB,lymphocyte count,and TC levels in the malnutrition group were higher than before intervention(P<0.05).【Conclusion】Age,body mass index,ALB,lymphocyte count,and TC are independent risk factors that affect the risk of malnutrition in patients with chronic heart failure.Clinical attention should be given to them.At the same time,personalized nutritional support is beneficial for improving the nutritional status of patients with chronic heart failure,thereby promoting the improvement of prognosis.
