呼吸肌超声在麻醉管理中的应用进展

摘　　要：超声因无创、实时、价廉等优势已广泛用于麻醉、重症、呼吸等临床领域。呼吸肌超声主要是对膈肌、肋间肌、腹肌等重要呼吸肌进行评估,目前关于呼吸肌超声在围麻醉期中的应用尚未普及。本文结合近年来国内外研究文献,对呼吸肌超声的评价指标和在麻醉管理中的应用等进行综述。文献复习结果表明,呼吸肌超声对评估患者的呼吸功能具有良好的效果,在麻醉管理期间体现在术前评估患者肺功能以及术后评估脱机拔管时机、肌松残余及预测术后肺部并发症等。提示呼吸肌超声具有较高的临床价值,有望在围麻醉期进一步拓展应用。Ultrasound has the advantages of non-invasive,real-time and low cost,and has been widely used in clinic,especially in anesthesia,severe illness,respiratory and other clinical fields.Respiratory muscle ultrasound is mainly used to evaluate diaphragm,intercostal muscle,abdominal muscle and other important respiratory muscles.At present,the application of respiratory muscle ultrasound in perioperative anesthesia has not been popularized.Based on the research literature at home and abroad in recent years,this paper reviews the evaluation methods,indicators and application in anesthesia management of respiratory muscle ultrasound.The results of literature review show that respiratory muscle ultrasound has a good effect on evaluating patients'respiratory function,which is reflected in preoperative evaluation of patients'lung function,postoperative evaluation of offline extubation time,muscle relaxation residue and prediction of postoperative pulmonary complications during anesthesia management.These results show that respiratory muscle ultrasound has high clinical value and is expected to further expand its application during perioperative period.
