摘　　要：目的 探讨以家庭病床为基础的居家药学服务对2型糖尿病(T2DM)患者疗效及安全性的影响。方法 选取医院2023年6月至2024年6月收治的服用5种以上药品的T2DM患者206例,按随机数字表法分为对照组和研究组,各103例。两组患者均予常规药学服务,研究组患者增加9个月以家庭病床为基础的居家药学服务。结果 干预后,研究组患者的空腹血糖、糖化血红蛋白均显著低于对照组(P<0.05);糖尿病自我管理行为量表(DSMQ)及Morisky用药依从性量表(MMAS-8)评分均显著高于对照组(P<0.05)。干预过程中,研究组患者的低血糖和治疗相关不良反应发生率均显著低于对照组(9.71%比38.83%,15.53%比48.54%,P<0.05)。结论 以家庭病床为基础的居家药学服务用于T2DM患者的用药管理,可充分发挥药师的作用,提升患者的疗效及安全性。Objective To investigate the effect of home-based pharmaceutical care based on home-care beds on the treatment efficacy and safety of type 2 diabetes mellitus(T2DM).Methods A total of 206 T2DM patients who took more than five kinds of drugs and admitted to the hospital from June 2023 to June 2024 were selected and divided into the control group and the study group by the random number table method,with 103 cases in each group.The patients in the two groups received routine pharmaceutical care,while the patients in the study group additionally received home-based pharmaceutical care based on home-care beds for nine months.Results After the intervention,the fasting blood glucose and glycated hemoglob in the study group were significantly lower than those in the control group(P<0.05);the scores of the Diabetes Self-Management Questionnaire(DSMQ)and the 8-item Morisky Medication Adherence Scale(MMAS-8)in the study group were significantly higher than those in the control group(P<0.05).During the intervention process,the incidence rates of hypoglycemia and treatment-related adverse reactions in the study group were significantly lower than those in the control group(9.71%vs.38.83%,15.53%vs.48.54%,P<0.05).Conclusion Home-based pharmaceutical care based on home-care beds can fully utilize the role of pharmacists in the management of medication for T2DM patients,improve the treatment efficacy and safety of patients.
