双歧杆菌四联活菌片联合吡美莫司乳膏治疗特应性皮炎临床研究

摘　　要：目的探讨双歧杆菌四联活菌片联合吡美莫司乳膏治疗特应性皮炎(AD)的临床疗效,以及对患者皮肤屏障功能及免疫功能的影响。方法选取江苏省常熟市第一人民医院2020年10月至2022年10月收治的AD患者123例,按随机数字表法分为对照组(62例)和观察组(61例)。研究期间,观察组失访1例,对照组失访2例,最终两组均纳入60例患者。两组患者均予吡美莫司乳膏治疗,观察组患者加用双歧杆菌四联活菌片。两组患者均连续治疗2个月,随访1年。结果观察组患者总有效率为96.67%,显著高于对照组的78.33%(P<0.05)。治疗后,与对照组比较,观察组患者的湿疹面积及严重程度指数(EASI)评分、皮肤病生活质量指数(DLQI)评分均显著更低(P<0.05);角质层含水量、皮脂含量均显著更高(P<0.05),经皮水分丢失(TEWL)显著更低(P<0.05);CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)均显著更高(P<0.05),CD_(8)^(+)显著更低(P<0.05);血清γ干扰素、白细胞介素4、白细胞介素13水平均显著更低(P<0.05);双歧杆菌、乳酸杆菌数量均显著更多(P<0.05),肠球菌、肠杆菌数量均显著更少(P<0.05)。观察组患者1年内复发率为1.67%,显著低于对照组的15.00%(P<0.05)。结论双歧杆菌四联活菌片联合吡美莫司乳膏治疗AD的疗效较好,可有效缓解患者的湿疹、皮损等症状,改善皮肤屏障功能及免疫功能,纠正Th1/Th2细胞失衡,调节肠道菌群,降低复发率。Objective To investigate the clinical efficacy of Bifidobacterium Quadruplex Viable Tablets combined with Pimecrolimus Cream in the treatment of atopic dermatitis(AD),and its effect on the skin barrier function and immune function of patients.Methods A total of 123 AD patients admitted to the First People's Hospital of Changshu from October 2020 to October 2022 were selected and randomly divided into the control group(62 cases)and the observation group(61 cases)by the random number table method.During the study,one case was lost to follow-up in the observation group and two cases were lost to follow-up in the control group.Ultimately,60 cases were included in each group.The patients in the two groups were treated with Pimecrolimus Cream,while the patients in the observation group were treated with Bifidobacterium Quadruplex Viable Tablets.Both groups were treated continuously for two months and followed up for one year.Results The total effective rate in the observation group was 96.67%,which was significantly higher than 78.33%in the control group(P<0.05).After treatment,compared with those in the control group,the scores of Eczema Area and Severity Index(EASI)and Dermatology Quality of Life Index(DLQI)(P<0.05)in the observation group were significantly lower,the moisture content and sebum content of the stratum corneum in the observation group were significantly higher(P<0.05),while the transcutaneous water loss(TEWL)in the observation group was significantly lower(P<0.05);the CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher(P<0.05),while the CD_(8)^(+)in the observation group was significantly lower(P<0.05);the levels of serum interferon-γ,interleukin-4,and interleukin-13 in the observation group were significantly lower(P<0.05);the numbers of Bifidobacterium and Lactobacillus in the observation group were significantly higher(P<0.05),while the numbers of Enterococcus and Escherichia coli in the observation group were significantly lower(P<0.05).The recurrence rate within
