“三大中心”建设视角下上海市基层医疗卫生机构服务质量提升与服务场景建设构想

摘　　要：针对当前上海市提出的社区健康管理中心、社区康复中心、社区护理中心建设要求,但基层医疗卫生机构资源却相对有限的现状,思考如何通过合理配置、精准布局,最大化利用现有资源,减少试错,以快速明确社区“三大中心”建设路径,最终通过梳理社区“三大中心”的功能定位与基层医疗卫生机构实际工作场景,明确了社区健康管理中心、社区康复中心、社区护理中心各自在机构发展中的站位,三者的业务服务输出顺序,以及“中心—站点”双层服务模式。在这一模式下,基层医疗卫生机构两大服务输出层级各自明确其角色,共同发力,打造了“中心研发—站点服务”的建设模式,并基于现实服务工作场景对“三大中心”所需技术展开讨论,构建了三者共同发挥作用的,能够为居民带来更加高效、优质且便利的健康解决方案。In view of the current situation of primary medical and health institutions in Shanghai facing the construction requirements of“community health management center”,“community rehabilitation center”and“community care center”,but with relatively limited institutional resources,the study considered how to maximize the use of existing resources through reasonable allocation and accurate layout,reduce trial and error,and quickly clarify the construction path of the three major community centers.Finally,through combing the functional positioning of the three major community centers and the actual working scenarios of primary medical and health institutions,the positions of“community health management center”,“community rehabilitation center”and“community care center”in the development of institutions,the order of business service output of the three,and the center-site double-layer service mode were clarified.Under the mode,the two service output levels of primary medical and health institutions each clearly defined their roles,and worked together to create a center research and development-site service construction mode.
