妊娠合并抗磷脂抗体综合征的研究进展

摘　　要：抗磷脂抗体综合征是由抗磷脂抗体引起的一种临床体征、症状的疾病总称,临床表现为反复不良妊娠或静脉、动脉血栓,疾病特点为抗磷脂抗体出现持续的中高效价阳性。抗磷脂抗体是以内皮细胞膜、血小板上负电荷磷脂为靶抗原的一种自身抗体。抗磷脂抗体包括抗心磷脂抗体、狼疮凝血因子及抗β2糖蛋白抗体-1。抗磷脂抗体对妊娠影响较多且较为复杂,不同抗磷脂抗体阳性可预测不同不良妊娠结局,临床上对抗磷脂抗体综合征的诊断多采用实验室标准或Sapporo标准中一项进行诊断。妊娠合并抗磷脂抗体综合征患者临床上的治疗目标为提高新生儿活产率,改善母胎并发症。产科抗磷脂抗体综合征治疗包括抗凝治疗、免疫调节治疗(糖皮质激素、羟氯喹、维生素D、免疫球蛋白、他汀类药物及利妥昔单抗)。本文对妊娠合并抗磷脂抗体综合征的研究进展进行综述,以期为妊娠合并抗磷脂抗体综合征的临床治疗提供参考。Antiphospholipid antibody syndrome is a general term for clinical signs and symptoms caused by antiphospholipid antibodies.Clinical manifestations include recurrent adverse pregnancies or venous and arterial thrombosis,and the disease is characterized by sustained high titer positivity of antiphospholipid antibodies.Antiphospholipid antibodies are autoantibodies that target negatively charged phospholipids on endothelial cell membranes and platelets.Antiphospholipids antibody include anti cardiolipin antibodies,lupus coagulation factors and anti-β2 glycoprotein anti-1 antibodies.Antiphospholipid antibodies have a significant and complex impact on pregnancy.Positive results of different antiphospholipid antibodies can predict different adverse pregnancy outcomes.In clinical practice,the diagnosis of antiphospholipid antibody syndrome is often based on one of the laboratory or Sapporo criteria.The clinical treatment goal for patients with pregnancy complicated with antiphospholipid antibody syndrome is to increase the live birth rate of newborns and improve maternal and fetal complications.The treatment of obstetric antiphospholipid antibody syndrome includes anticoagulant therapy,immunomodulatory therapy(glucocorticoids,hydroxychloroquine,vitamin D,immunoglobulin,statins and rituximab).This article reviews the research progress on pregnancy complicated with antiphospholipid antibody syndrome,in order to provide reference for the clinical treatment of pregnancy complicated with antiphospholipid antibody syndrome.
