利妥昔单抗治疗膜性肾病的临床疗效观察与护理

摘　　要：目的观察利妥昔单抗临床治疗膜性肾病(MN)的疗效,探索其护理干预策略。方法选择天津医科大学第二医院肾内科2023年1月—2024年1月治疗的134例MN患者作为研究对象,按周期输注利妥昔单抗,观察临床疗效及不良反应率。结果使用利妥昔单抗后,部分缓解和全部缓解的患者98例,有效缓解率为73.13%,说明利妥昔单抗能较好地促进病情缓解。在治疗中,不良反应有63例,占总患者数的47.01%。不良反应主要为输液相关反应32例,胃肠道异常24例及焦虑或抑郁15例,分别占不良反应患者总例数的50.79%、38.10%、23.81%。不良反应发生后,护理人员根据不同的反应给出不同的护理方案,症状出现了好转。结论利妥昔单抗临床治疗MN能提高患者的缓解率,护理人员有效的干预护理,照顾到了患者身心健康,提供舒适、安全可靠及专业的治疗氛围,使得研究顺利完成,为今后采用利妥昔单抗药物的护理实践提供了有价值的参考。Objective To observe the clinical efficacy of rituximab in the treatment of membranous nephropathy(MN)and explore its nursing intervention strategies.Methods A total of 134 patients with MN treated in the Department of Nephrology in Second Hospital of Tianjin Medical University from January2023 to January 2024 were selected as the study subjects,and rituximab was injected periodically to observe their clinical efficacy,and adverse reaction rate.Results After treatment with rituximab,98patients showed partial and complete remission,with a remission rate of 73.13%,indicating that rituximab had a good therapeutic effect in inducing remission.During the treatment,63 patients experienced adverse reactions,with an adverse reaction rate of 47.01%.The main adverse reactions were infusion related reactions in 32 cases,gastrointestinal abnormalities in 24 cases,and anxiety or depression in 15 cases,accounting for 50.79%,38.10%and 23.81%of the total number of adverse reaction patients,respectively.After the occurrence of adverse reactions,nursing staff provided symptomatic treatment according to different reactions,and symptoms improved.Conclusion The treatment of MN with rituximab could improve the clinical remission rate of patients.The effective intervention nursing of nursing staff could care for the physical and mental health of patients,providing a relaxed,safe reliable and professional treatment environment for patients,and facilitating the smooth completion of the study.This provided valuable reference for the nursing practice of using rituximab drugs in the future.
