摘　　要：目的 通过回顾性研究伏立康唑谷浓度,探索肺部真菌感染患者中质子泵抑制剂对伏立康唑谷浓度的影响。方法 收集2022年6月~2023年12月在泉州第一医院使用伏立康唑进行治疗的患者信息。汇总患者的一般资料、质子泵抑制剂给药方案、伏立康唑给药方案、其他存在相互作用合并用药方案及伏立康唑治疗药物监测数据。根据伏立康唑、质子泵抑制剂的联合用药情况分为伏立康唑单药组和伏立康唑联用质子泵抑制剂组;分析两组伏立康唑谷浓度的差异,以及影响伏立康唑谷浓度的影响因素。结果 对比两组的伏立康唑谷浓度,联合用药组伏立康唑谷浓度的中位数略高于单药组,但两组伏立康唑谷浓度无统计学差异。对质子泵抑制剂种类进行分层分析,发现奥美拉唑、雷贝拉唑与伏立康唑联用时,奥美拉唑组伏立康唑谷浓度值中位数高于雷贝拉唑组;同时,联合用药组中亚治疗浓度的占比低于单药组。结论 质子泵抑制剂能够在一定程度上提升伏立康唑谷浓度。OBJECTIVE The effect of proton pump inhibitors on voriconazole valley concentration in patients with pulmonary fungal infection was studied retrospectively.METHODS Information was collected from patients treated with voriconazole at Izumi between June 2022 and December 2023.The general data,proton pump inhibitor administration schedule,voriconazole administration schedule,other drug regimens with interaction and voriconazole treatment monitoring data were summarized.According to the combination of voriconazole and proton pump inhibitor,they were divided into voriconazole monotherapy group and voriconazole combined proton pump inhibitor group.The difference of voriconazole valley concentration between the two groups and the influencing factors of voriconazole valley concentration were analyzed.RESULTS Comparing the voriconazole valley concentration between the two groups,the median voriconazole valley concentration in the combination group was slightly higher than that in the monotherapy group,but there was no statistical difference between the two groups.By stratified analysis of the types of proton pump inhibitors,it was found that the median voriconazole valley concentration in omeprazole group was higher than that in rabeprazole group when omeprazole,rabeprazole and voriconazole were combined.Meanwhile,the proportion of subtherapeutic concentration in the combined drug group was lower than that in the single drug group.CONCLUSION Proton pump inhibitors can increase voriconazole valley concentration to some extent.
