碳青霉烯耐药肺炎克雷伯菌的临床分布及其检出率与抗菌药物使用强度占比的相关性分析

摘　　要：目的通过回顾性分析肺炎克雷伯菌感染率、CRKP检出率与抗菌药物使用强度占比的关系,为防控CRKP和加强抗菌药物合理使用提供依据。方法采用Pearson相关性统计方法分析抗菌药物AUD和AUD%与KPN感染率和CRKP检出率之间的关系。结果CRKP主要集中在重症医学科(37.3%)和神经外科(35.1%)。KPN感染率与抗菌药物使用强度P<0.01以及碳青霉烯类P<0.01、单环类P<0.05、四环素类P<0.05、抗阳性菌药P<0.01、粘菌素类P<0.01的AUD%正相关。CRKP检出率与碳青霉烯类(P<0.05)、氨基糖苷类(P<0.01)、四环素类(P<0.05)、抗阳性菌药(P<0.01)的AUD%正相关,与喹诺酮类的AUD%负相关(P<0.05)。结论重症医学科和神经外科是CRKP检出的主要科室,需对碳青霉烯类、氨基糖苷类、四环素类和抗阳性菌药的临床使用加强监控。OBJECTIVE To retrospectively analyze the relationship between the infection rate of Klebsiella pneumoniae,the detection rate of CRKP,and the proportion of antibiotic use intensity,in order to provide a support for preventing and controlling infection of CRKP and strengthening the rational use of antibiotics.METHODS Use Pearson correlation statistical method to analyze the relationship between antibiotic AUD and AUD%and KPN infection rate and CRKP detection rate.RESULTS CRKP is mainly concentrated in intensive care medicine(37.3%)and neurosurgery(35.1%).The KPN infection rate is positively correlated with the intensity of antibacterial drug use P<0.01,as well as the AUD%of carbapenems P<0.01,monocyclic antibiotics P<0.05,tetracyclines P<0.05,anti-positive bacterial drugs P<0.01,and colistin P<0.01.The detection rate of CRKP is positively correlated with the AUD%of carbapenems(P<0.05),aminoglycosides(P<0.01),tetracyclines(P<0.05),and anti-positive bacterial drugs(P<0.01),and negatively correlated with the AUD%of quinolones(P<0.05).CONCLUSION Intensive care medicine and neurosurgery are the main departments of CRKP.Carbapenems,aminoglycosides,tetracyclines,and anti-positive bacterial drugs can induce CRKP production,and their clinical use needs to be closely monitored.
