医共体模式下Triangle管理在基层医院重点监控药品成效分析中的应用

摘　　要：目的分析“医共体”模式下采用Triangle分层分级管理模式在2022至2023年期间对南京市高淳区4家基层医疗机构重点监控药品管控效果,促进基层医院“医共体”的合理用药。方法对Triangle分层管理前后监控药品用量金额、人均金额、用药占比、抗菌药物使用率、使用强度和合理用药点评情况进行比较分析。结果临床药师通过Triangle分层分级模式实现了重点监控药品的有效管理,管控后重点监控药品金额下降8.16%,人均金额下降16.54%,监控药品金额占比下降15.38%;药师点评效率由每小时6.06份提高到每小时8.33份,争议病历数由67份下降至21份,不合格病历由43份下降至23份。结论在“医共体”模式下通过Triangle分层分级管理模式用于重点监控药品管控的效果显著,在有限的专科临床药师基础上能够实现的区域内医疗机构的优化配置,促进临床合理用药。OBJECTIVE To analyze the effects of the Triangle hierarchical management model on the control and management of key medicines in 4 primary healthcare institutions in Gaochun District,Nanjing City,from 2022 to 2023 under the model of medical community,and to promote the rational use of medicines in primary care hospitals under the model of medical community reasonable use of medicines.METHODS A comparative analysis was conducted on the monitored drug dosage amount,per capita amount,medication proportion,antibacterial drug usage rate,usage intensity and rational drug use comments before and after Triangle hierarchical model.RESULTS Clinical pharmacists achieved effective management of key monitored drugs through the Triangle hierarchical model,resulting in an 8.16%decrease in the amount of key monitored drugs,a 16.54%decrease in per capita amount,and a 15.38%decrease in the proportion of monitored drug costs.The efficiency of pharmacist reviews increased from 6.06 to 8.33 cases per hour,the number of disputed cases decreased from 67 to 21,and the number of non-compliant cases reduced from 43 to 23.CONCLUSION Under the medical community model,the Triangle hierarchical management model significantly monitors critical medicines′control.Based on a limited number of specialized clinical pharmacists,it can achieve the optimal allocation of medical institutions in the region and promote the rational use of medicines in the clinic.
