摘　　要：目的 探讨医疗失效模式与效应分析应用于门诊儿童用药供应保障流程的改善效果。方法 选取某市三甲医院儿童院区门诊药房2022年第一季度和第二季度药品领药情况,运用HFMEA法,分析门诊儿童用药供应保障流程中的风险点,进行失效模式与危害分析,制定改善策略并进行效果评价。结果 改善后紧缺药品缺药率、因系统限制所致的医嘱失败率、儿童特需药品入账周期、未切换药房导致取药失败频次、医生反映缺药频次均低于改善前,改善后厂家更换知晓率高于改善前,医疗失效模式与效应分析的风险值(RPN)也明显下降。结论 运用HFMEA可以有效改善儿童门诊用药供应保障流程,减少缺药频次,保证儿童得到及时有效的药物治疗。OBJECTIVE To investigate the effect of medical failure and effect model in the improvement of the drug supply guarantee process for outpatient children.METHODS The drug collection situation in the first and second quarters of 2022 was selected from the outpatient pharmacy of the children′s hospital of a tertiary hospital in a city,and the HFMEA method was used to analyze the risk points in the process of ensuring the supply of drugs for outpatient children,and the failure mode and hazard analysis were carried out,and the improvement strategies were formulated and the effect evaluation was carried out.RESULTS The drug shortage rate of drugs in short supply after the improvement,the failure rate of doctor′s orders due to system limitations,the billing cycle of children′s special drugs,the frequency of drug failure due to failure to switch pharmacies,and the frequency of drug shortage reported by doctors were all lower than before the improvement,and the awareness rate of manufacturer replacement after improvement was higher than that before the improvement,and the risk value(RPN)of medical failure mode and effect analysis also decreased significantly.CONCLUSION The use of HFMEA can effectively improve the process of ensuring the supply of outpatient medication for children,reduce the frequency of drug shortage,and ensure that children receive timely and effective drug treatment.
