空腹胰岛素和C肽联合检测在糖尿病诊断中的效果分析

摘　　要：目的分析在糖尿病(diabetes mellitus,DM)诊断中,空腹胰岛素与C肽联合检测的应用价值及效果。方法选取2021年1月—2023年12月南靖县中医院收治的80例疑似DM患者为研究对象,均进行空腹胰岛素、C肽检测,以口服葡萄糖耐量试验(oral glucose tolerance test,OGTT)为金标准,统计OGTT诊断结果、C肽与空腹胰岛素单独检测及联合检测结果,比较两者单独及联合检测DM的诊断效能。结果80例疑似DM患者经OGTT诊断,阳性62例,阴性18例。C肽检测真阳性53例、真阴性13例,空腹胰岛素检测真阳性51例、真阴性10例,联合检测真阳性61例、真阴性16例。联合检测的灵敏度、准确度、阴性预测值均高于C肽与空腹胰岛素单独检测,差异均有统计学意义(P均<0.05)。结论DM患者经空腹胰岛素、C肽联合检测,其灵敏度、准确度、阴性预测值更高,能为DM诊断、治疗提供有效依据。Objective To analyze the application value and effect of combined detection of fasting insulin and Cpeptide in the diagnosis of diabetes mellitus(DM).Methods A total of 80 patients with suspected DM admitted to Nanjing County Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were selected as the research objects.Fasting insulin and C-peptide were detected.Oral glucose tolerance test(OGTT)was used as the gold standard.The results of OGTT diagnosis,C-peptide and fasting insulin detection alone and combined detection were statistically analyzed.The diagnostic efficacy of single detection and combined detection of DM was compared.Results Eighty patients with suspected DM were diagnosed by OGTT,62 cases were positive and 18 cases were negative.C-peptide detection was true positive in 53 cases,true negative in 13 cases,fasting insulin detection was true positive in 51 cases,true negative in 10 cases,combined detection was true positive in 61 cases,true negative in 16 cases.The sensitivity,accuracy and negative predictive value of combined detection were higher than those of Cpeptide and fasting insulin alone,and the differences were statistically significant(all P<0.05).Conclusion The combined detection of fasting insulin and C-peptide in DM patients has higher sensitivity,accuracy and negative predictive value,which can provide an effective basis for the diagnosis and treatment of DM.
