摘 要:目的探讨儿童及青少年1型糖尿病(type 1 diabetes mellitus,T1DM)糖化血红蛋白水平(glycated hemoglobin A1c,HbA1c)与空腹血糖、空腹C肽、血脂水平的相关性。方法回顾性选取2018年1月—2023年6月泉州市儿童医院小儿内分泌科收治的138例T1DM患儿的临床资料,根据HbA1c水平将其分为≤7%组(55例)、7%<~8%组(34例)、8%<~<9%组(29例)及>9%组(20例),比较各组空腹血糖、空腹C肽、血脂指标的差异,并分析上述指标与HbA1c水平的相关性。结果不同HbA1c水平的T1DM患儿空腹血糖、空腹C肽、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、载脂蛋白A、载脂蛋白B、载脂蛋白A/载脂蛋白B水平比较,差异均有统计学意义(P均<0.05)。总胆固醇、低密度脂蛋白胆固醇及空腹血糖水平与T1DM患儿HbA1c水平呈正相关,高密度脂蛋白胆固醇、载脂蛋白A、载脂蛋白B、载脂蛋白A/载脂蛋白B及空腹C肽水平与T1DM患儿HbA1c水平呈负相关。结论强化血糖管理、监测血脂水平有助于改善儿童及青少年T1DM的血脂紊乱。Objective To investigate the correlation between glycated hemoglobin A1c(HbA1c)and fasting plasma glucose,fasting C-peptide and blood lipid level in children and adolescents with type 1 diabetes mellitus(T1DM).Methods The clinical data of 138 children with T1DM admitted to the Department of Pediatric Endocrinology of Quanzhou Children's Hospital from January 2018 to June 2023 were retrospectively selected.According to the level of HbA1c,they were divided into≤7%group(55 cases),7%<~8%group(34 cases),8%<~<9%group(29 cases)and>9%group(20 cases).The differences of fasting plasma glucose,fasting C-peptide and blood lipid indexes in each group were compared,and the correlation between above indexes and the HbA1c level was analyzed.Results There were statistically significant differences in the levels of fasting plasma glucose,fasting C-peptide,total cholesterol,triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,apolipoprotein A,apolipoprotein B,and apolipoprotein A/apolipoprotein B in children with T1DM at different HbA1c levels(all P<0.05).The levels of total cholesterol,low-density lipoprotein cholesterol and fasting plasma glucose were positively correlated with HbA1c levels in children with T1DM.The levels of high-density lipoprotein cholesterol,apolipoprotein A,apolipoprotein B,apolipoprotein A/apolipoprotein B and fasting C-peptide were negatively correlated with HbA1c levels in children with T1DM.Conclusion Strengthening blood glucose management and monitoring blood lipid levels are helpful to improve dyslipidemia in children and adolescents with T1DM.
