胰岛素泵短期强化治疗对新发2型糖尿病胰岛素分泌、敏感性的影响

摘　　要：目的探究新发2型糖尿病患者应用胰岛素泵短期强化治疗对胰岛素分泌及敏感性的影响。方法选择2023年1—12月济南市第七人民医院社区卫生服务中心收治新发2型糖尿病患者60例,按不同治疗方案分组。对照组(n=30)采用胰岛素皮下注射治疗,观察组(n=30)接受胰岛素泵短期强化治疗。比较两组患者胰岛素分泌与敏感性、血糖指标、不良反应发生率、空腹胰岛素及空腹C肽指标。结果治疗后,与对照组相比,观察组胰岛β细胞功能指数、空腹C肽更高,胰岛素抵抗指数、空腹血糖、餐后2 h血糖、糖化血红蛋白、空腹胰岛素水平更低,差异均有统计学意义(P均<0.05)。两组不良反应总发生率对比,差异无统计学意义(P>0.05)。结论胰岛素泵短期强化治疗新发2型糖尿病患者的效果更加显著,可帮助患者平稳血糖指标,改善胰岛素敏感性,减轻胰岛素抵抗情况,同时安全性较高。Objective To explore the effect of short-term intensive insulin pump therapy on insulin secretion and sensitivity in newly diagnosed type 2 diabetes mellitus.Methods From January to December 2023,60 patients with newdiagnosed type 2 diabetes mellitus admitted to the Jinan Seventh People's Hospital Community Health Service Center were selected as the research objects,and they were divided into two groups according to different treatment methods.The control group(n=30)was treated with subcutaneous injection of insulin,and the observation group(n=30)was treated with short-term intensive insulin pump therapy.Insulin secretion and sensitivity,blood glucose indexes,incidence of adverse reactions,fasting insulin and fasting C peptide were compared between the two groups.Results After treatment,compared with the control group,the isletβ-cell function index and fasting C-peptide in the observation group were higher,and the insulin resistance index,fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin A1c and fasting insulin levels were lower,the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The effect of short-term intensive insulin pump therapy on patients with newly diagnosed type 2 diabetes mellitus is more significant,which can help patients stabilize blood glucose indicators,improve insulin sensitivity,reduce insulin resistance,and have higher safety.
