达格列净联合二甲双胍治疗血糖控制不佳的老年2型糖尿病患者的疗效分析

摘　　要：目的探讨血糖控制不佳的老年2型糖尿病患者应用二甲双胍联合达格列净治疗的临床效果。方法回顾性选取2022年6月—2024年6月北京市社会福利医院收治的100例血糖控制不佳的老年2型糖尿病患者的临床资料,根据不同的治疗方法分为两组,各50例。两组均进行门冬胰岛素治疗,对照组进行二甲双胍治疗,观察组在对照组的基础上进行达格列净治疗。比较两组血糖控制效果、胰岛功能指标、氧化应激指标、不良反应发生率。结果治疗后,与对照相比,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、空腹胰岛素、丙二醛水平、胰岛素抵抗指数均更低,超氧化物歧化酶、总抗氧化能力均更高,差异均有统计学意义(P均<0.05)。在不良反应发生率方面,两组比较,差异无统计学意义(P>0.05)。结论老年2型糖尿病患者进行二甲双胍联合达格列净治疗,可发挥减轻胰岛素抵抗、缓解氧化应激反应的显著作用,血糖控制效果及安全性好。Objective To investigate the clinical effect of metformin combined with dapagliflozin in elderly patients with type 2 diabetes mellitus with poor blood glucose control.Methods The clinical data of 100 elderly type 2 diabetes mellitus patients with poor blood glucose control admitted to Beijing Social Welfare Hospital from June 2022 to June 2024 were retrospectively selected and divided into two groups according to different treatment methods,50 cases in each group.Both groups were treated with insulin aspart,the control group was treated with metformin,and the observation group was treated with dapagliflozin on the basis of the control group.The blood glucose control effect,islet function indexes,oxidative stress indexes and incidence of adverse reactions were compared between the two groups.Results After treatment,compared with the control group,the fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin A1c,fasting insulin,malondialdehyde level and insulin resistance index in the observation group were lower,while the superoxide dismutase and total antioxidant capacity were higher,the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of metformin combined with dapagliflozin in elderly patients with type 2 diabetes mellitus can play a significant role in reducing insulin resistance and alleviating oxidative stress response.The blood glucose control effect and the safety were good.
