摘  要:目的探究微量注射泵胰岛素输注在糖尿病酮症酸中毒(diabetic ketoacidosis,DKA)重症抢救中的应用效果及对低血糖发生风险的影响。方法回顾性选取2023年4月—2024年3月期间在泉州市第一医院进行治疗的124例DKA患者的临床资料,凭借治疗方法的不同分为常规组(n=62,持续静脉滴注胰岛素治疗)、观察组(n=62,微量注射泵胰岛素输注治疗)。比较两组临床指标、糖代谢水平及临床疗效。结果观察组血酮体恢复时间、pH值恢复时间、尿酮体恢复时间均短于常规组,低血糖发生次数少于常规组;治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于常规组,观察组治疗总有效率高于常规组,差异均有统计学意义(P均<0.05)。结论DKA患者在重症抢救中选择微量注射泵胰岛素输注方式,可减少低血糖反应发生次数,临床指标及糖代谢水平均得到显著改善,疗效显著。Objective To explore the application effect of microinjection pump insulin infusion in the severe rescue of diabetic ketoacidosis(DKA)and its influence on the risk of hypoglycemia.Methods The clinical data of 124 patients with DKA admitted to the Quanzhou First Hospital from April 2023 to March 2024 were retrospectively selected.According to the different treatment methods,they were divided into routine group(n=62,continuous intravenous infusion of insulin)and observation group(n=62,microinjection pump insulin infusion).The clinical indexes,glucose metabolism levels and clinical efficacy were compared between the two groups.Results The recovery time of blood ketone body,pH value and urine ketone body in the observation group were shorter than those in the routine group,and the number of hypoglycemia was less than that in the routine group,after treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the observation group were lower than those in the routine group,the total effective rate of treatment in the observation group was higher than that in the routine group,and the differences were statistically significant(all P<0.05).Conclusion DKA patients choose microinjection pump insulin infusion in severe rescue,which can reduce the number of hypoglycemia reactions,significantly improve clinical indicators and glucose metabolism levels,and have a significant effect.
