摘　　要：目的探究老年2型糖尿病(type 2 diabetes mellitus,T2DM)合并脑震荡患者控糖治疗中应用达格列净与胰岛素强化治疗的作用价值。方法回顾性选取2023年1月—2024年5月在中国人民解放军陆军第七十三集团军医院接受诊疗的116例老年T2DM合并脑震荡患者的临床资料,根据不同的治疗方法分为两组。常规组(58例)采用胰岛素强化治疗,观察组(58例)采用胰岛素强化治疗联合达格列净治疗。比较两组血糖水平、血糖波动情况、临床疗效。结果治疗3个月后,与对照组比较,观察组糖化血红蛋白、空腹血糖、餐后2 h血糖水平、血糖标准差、平均血糖水平、血糖波动最大幅度均较常规组更低,差异均有统计学意义(P均<0.05)。与常规组比较,观察组临床疗效更高,差异有统计学意义(P<0.05)。结论老年T2DM合并脑震荡患者治疗中应用达格列净联合胰岛素强化治疗,可提升控糖效果,改善血糖波动情况,临床疗效更佳。Objective To explore the value of dapagliflozin and intensive insulin therapy in the treatment of glucose control in elderly patients with type 2 diabetes mellitus(T2DM)complicated with cerebral concussion.Methods The clinical data of 116 elderly patients with T2DM complicated with cerebral concussion who were treated in 73rd Army Group Hospital of the PLA from January 2023 to May 2024 were retrospectively selected and divided into two groups according to different treatment methods.The conventional group(58 cases)was treated with intensive insulin therapy,and the observation group(58 cases)was treated with intensive insulin therapy combined with dapagliflozin.The blood glucose levels,blood glucose fluctuation and clinical efficacy were compared between the two groups.Results After 3 months of treatment,compared with the conventional group,the glycated hemoglobin A1c,fasting blood glucose,2-hour postprandial blood glucose levels,blood glucose standard deviation,average blood glucose level and the maximum amplitude of blood glucose fluctuation in the observation group were lower,and the differences were statistically significant(all P<0.05).Compared with the conventional group,the clinical efficacy of the observation group was higher,and the difference was statistically significant(P<0.05).Conclusion The application of dapagliflozin combined with intensive insulin therapy in the treatment of elderly patients with T2DM complicated with cerebral concussion can improve the effect of glucose control,improve the fluctuation of blood glucose,and the clinical effect is better.
