达格列净对糖尿病肾病患者血管内皮功能、氧化应激和血糖代谢的影响

摘  要:目的探讨达格列净对糖尿病肾病(diabetic nephropathy,DN)患者血管内皮功能、氧化应激和血糖代谢的影响。方法选取2023年5月—2024年5月贵黔国际总医院收治的300例DN患者,根据不同治疗方法将患者分为对照组(n=150,接受常规药物治疗)和达格列净组(n=150,达格列净联合常规药物治疗),均持续治疗24周。比较两组患者治疗前后血管内皮功能、氧化应激指标及血糖水平。观察两组治疗期间不良反应情况。结果治疗24周后,两组内皮素、血管性血友病因子水平均降低,且达格列净组均低于对照组,差异均有统计学意义(P均<0.05)。治疗24周后,达格列净组丙二醛水平低于对照组,超氧化物歧化酶水平高于对照组,差异均有统计学意义(P均<0.05)。治疗24周后,两组糖化血红蛋白、空腹血糖、餐后2 h血糖水平均降低,且达格净列组均低于对照组,差异均有统计学意义(P均<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论达格列净能够显著改善DN患者的血管内皮功能,降低患者的氧化应激水平,有效控制血糖代谢。Objective To investigate the effects of dapagliflozin on vascular endothelial function,oxidative stress and blood glucose metabolism in patients with diabetic nephropathy(DN).Methods A total of 300 patients with DN diagnosed in Guiqian International Hospital from May 2023 to May 2024 were selected and divided into control group(n=150,receiving conventional drug therapy)and dapagliflozin group(n=150,dapagliflozin combined with conventional drug therapy)according to different treatment methods,both of which were treated for 24 weeks.Vascular endothelial function,oxidative stress index and blood glucose level were compared between the two groups before and after treatment.The adverse reactions of the two groups during treatment were observed.Results After 24 weeks of treatment,the levels of endothelin and von willebrand factor in the two groups decreased,and those in the dapagliflozin group were lower than the control group,the differences were statistically significant(both P<0.05).After 24 weeks of treatment,the level of malondialdehyde in the dapagliflozin group was lower than that in the control group,and the level of superoxide dismutase was higher than that in the control group,the differences were statistically significant(both P<0.05).After 24 weeks of treatment,the levels of glycated hemoglobin A1c,fasting plasma glucose and 2-hour postprandial plasma glucose in the two groups decreased,and those in the dapagliflozin group were lower than the control group,the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Dapagliflozin can significantly improve the vascular endothelial function,reduce the level of oxidative stress and effectively control blood glucose metabolism in patients with DN.
