摘　　要：目的探讨2型糖尿病肾病(type 2 diabetic nephropathy,T2DN)患者联合使用达格列净、利拉鲁肽与二甲双胍的疗效与安全性。方法选取2021年1月—2024年1月期间在松滋市第三人民医院治疗的T2DN患者181例,依据不同治疗方法分为两组。对照组(n=90)采用达格列净联合二甲双胍=治疗,探究组(n=91)在对照组基础上进行利拉鲁肽治疗。比较两组血糖水平、症状积分及不良反应发生率。结果治疗后,探究组糖化血红蛋、餐后2 h血糖及空腹血糖水平均低于对照组,探究组蛋白尿、水肿、高血压、少尿症状积分均低于对照组,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论达格列净、二甲双胍与利拉鲁肽联合治疗T2DN患者,能够显著降低患者的血糖水平,改善其临床症状,且安全性良好。Objective To discuss the clinical efficacy and safety of dapagliflozin,liraglutide and metformin in patients with type 2 diabetic nephropathy(T2DN).Methods A total of 181 patients with T2DN received by the Songzi City Third People's Hospital from January 2021 to January 2024 were selected and divided into two groups according to different treatment methods.The control group(n=90)was treated with dapagliflozin combined with metformin,and the study group(n=91)was treated with liraglutide on the basis of the control group.The blood glucose level,symptom score and incidence of adverse reactions were compared between the two groups.Results After treatment,the levels of glycated hemoglobin A1c,2-hour postprandial plasma glucose and fasting plasma glucose in the study group were lower than those in the control group,the scores of proteinuria,edema,hypertension and oliguria in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of dapagliflozin,metformin and liraglutide in the treatment of T2DN patients can significantly reduce the blood glucose level of patients,improve their clinical symptoms,and have good safety.
