地塞米松耳后骨膜下注射对神经性耳鸣患者的临床疗效

摘　　要：目的观察地塞米松耳后骨膜下注射对神经性耳鸣患者的临床疗效。方法170例神经性耳鸣患者随机分为两组,每组85例。对照组采用口服盐酸倍他司汀和甲钴胺片常规治疗,研究组在常规治疗基础上加用耳后骨膜下注射地塞米松,每次5mg,每3天1次,共注射5次。比较两组治疗前和治疗结束3周后耳鸣情况、听力阈值,观察治疗期间不良反应发生情况。结果治疗前两组耳鸣障碍量表(THI)评分、耳鸣评价量表(TEQ)评分以及高、低频听力阈值比较均无统计学差异(P>0.05)。治疗结束3周后,两组THI评分、TEQ评分以及高、低频听力阈值均低于治疗前,且研究组低于对照组(P<0.01)。研究组不良反应发生率稍高于对照组(P>0.05)。结论神经性耳鸣患者在常规治疗基础上加用地塞米松耳后骨膜下注射能显著提高临床疗效,改善耳鸣程度和听力阈值。Objective To observe the clinical effect of retroauricular subperiosteal injection of dexamethasone on patients with nervous tinnitus.Methods A total of 170 patients with nervous tinnitus were randomly divided into two groups with 85 cases each.Group C received conventional treatments including bethistine hydrochloride and methocobalamin tablets.On the basis of the treaments in group C,group A received additional retroauricular subperiosteal injection of dexamethasone 5mg once every 3 days for 5 times.The tinnitus and hearing threshold were compared between the two groups before and 3 weeks after treatment.The adverse reactions during the treatment period were observed.Results There were no significant differences in the scores of the tinnitus handicap inventory(THI),tinnitus evaluation questionnaire(TEQ),and high and low frequency hearing thresholds between the two groups before treatment(P>0.05).Three weeks after treatment,the THI score,TEQ score,and high and low frequency hearing thresholds were lower than those before treatment,which in group A were lower than those in group C(P<0.01).The incidence of adverse reactions in group A was slightly higher than that in group C(P>0.05).Conclusion On the basis of conventional treatments for the patients with nervous tinnitus,additional retroauricular subperiosteal injection of dexamethasone can improve the efficacy,alleviate tinnitus severity and hearing threshold.
