小儿豉翘清热颗粒联合利巴韦林治疗儿童手足口病Meta分析

摘　　要：目的 系统评价小儿豉翘清热颗粒治疗儿童手足口病的有效性和安全性。方法 检索PubMed、Embase、Web of Science、Cochrane Library、中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)和中国生物医学文献数据库(CBM)建库至2023年2月17日收录的利巴韦林(对照组)联用小儿豉翘清热颗粒(试验组)治疗儿童手足口病的随机对照试验(RCT),主要结局指标为临床有效率和不良反应发生率,次要结局指标为退热时间、皮疹消退时间、溃疡愈合时间、住院时间及其他实验室指标。采用Cochrane偏倚风险评估工具评价纳入研究的偏倚风险,使用RevMan5.2软件进行Meta分析。结果 共纳入15项RCT,包含1 758例患儿,其中试验组886例、对照组872例。Meta分析结果显示,在利巴韦林治疗手足口病基础上加用小儿豉翘清热颗粒,可提高患儿临床有效率[OR=3.96,95%CI (2.79,5.62),P<0.001],试验组和对照组之间不良反应差异无统计学意义[OR=0.43,95%CI(0.10,1.79),P=0.25]。结论 小儿豉翘清热颗粒治疗儿童手足口病疗效较好,且无显著不良反应,但受限于纳入研究数量和质量,需开展更多高质量RCT验证小儿豉翘清热颗粒治疗儿童手足口病有效性和安全性。Objective To systematically evaluate the efficacy and safety of Xiao’er Chiqiao Qingre Granules on hand,foot and mouth disease(HFMD)in children.Methods The randomized controlled trials(RCTs)for the treatment of HFMD with Xiao’er Chiqiao Qingre Granules combined with ribavirin(trial group)and ribavirin(control group)were retrieved from PubMed,Embase,Web of Science,Cochrane Library,CNKI,Wanfang Data and CBM databases from the establishment of the databases to February 17,2023.The primary outcomes were clinical effectiveness and incidence of adverse events,and secondary outcomes were time to fever resolution,time to rash resolution,time to ulcer healing,time to hospitalisation and other laboratory indicators.The risk of bias of the included studies was evaluated using the Cochrane risk of bias tool(ROB)and Meta-analysis was performed using RevMan5.2 software.Results A total of 1758 children were included in 15 studies,886 cases in trial group and 872 in control group.Meta-analysis results showed that on the basis of treating HFMD with ribavirin,the addition of Xiao’er Chiqiao Qingre Granules could improve the clinical effective rate of children[OR=3.96,95%CI(2.79,5.62),P<0.001].There was no difference in the adverse effects between the trial group and control groups[OR=0.43,95%CI(0.10,1.79),P=0.25].Conclusion Xiao’er Chiqiao Qingre Granules have good efficacy and no significant adverse effects with children with HFMD.However,limited by the number and quality of included studies,more high-quality RCTs are needed to validate the efficacy and safety of Xiao’er Chiqiao Qingre Granules in the treatment of HFMD.
