GnRH-a联合地诺孕素在卵巢巧克力囊肿术后维持治疗中的应用效果分析

摘　　要：目的:分析促性腺激素释放激素激动剂(GnRH-a)联合地诺孕素在卵巢巧克力囊肿术后维持治疗中的应用效果。方法:纳入卵巢巧克力囊肿患者100例为研究对象,采用随机数字表法分为常模组(50例)和研讨组(50例),前者在腹腔镜手术后采用GnRH-a治疗,后者在腹腔镜手术后采用GnRH-a联合地诺孕素治疗。对比两组的治疗总有效率、血清糖类抗原125(CA125)水平、痛经程度及不良反应发生率,术后随访12个月,记录两组病情复发率。结果:研讨组治疗总有效率高于常模组(P<0.05)。治疗后,研讨组视觉模拟评分法(VAS)评分、血清CA125水平均低于常模组(P<0.05)。两组不良反应发生率比较差异较小(P>0.05)。术后随访12个月,研讨组病情复发率4.00%,低于常模组的18.00%(P<0.05)。结论:GnRH-a联合地诺孕素在卵巢巧克力囊肿术后维持治疗中的应用效果显著,有利于改善患者血清CA125水平,减轻痛经程度,降低术后复发风险,安全有效。Objective:To evaluate the efficacy of gonadotropin-releasing hormone agonist(GnRH-a)combined with dienogest in postoperative maintenance therapy for ovarian chocolate cysts.Methods:A total of 100 patients with ovarian chocolate cysts were enrolled and randomly divided into a control group(n=50)and a combination group(n=50).The control group received GnRH-a monotherapy after laparoscopic surgery,while the combination group received GnRH-a plus dienogest.Therapeutic efficacy,serum carbohydrate antigen 125(CA125)levels,dysmenorrhea severity(assessed by Visual Analog Scale,VAS),incidence of adverse reactions,and 12-month postoperative recurrence rates were compared between the two groups.Results:The combination group exhibited a significantly higher total effective rate than the control group(P<0.05).Post-treatment VAS scores and serum CA125 levels in the combination group were markedly lower than those in the control group(P<0.05).No significant difference in adverse reaction rates was observed between groups(P>0.05).During the 12-month follow-up,the recurrence rate in the combination group(4.00%)was significantly lower than that in the control group(18.00%,P<0.05).Conclusion:GnRH-a combined with dienogest demonstrates significant efficacy in postoperative maintenance therapy for ovarian chocolate cysts.This regimen effectively reduces serum CA125 levels,alleviates dysmenorrhea,decreases recurrence risk,and maintains a favorable safety profile.
