超短波疗法结合股四头肌训练对膝骨关节炎合并滑膜炎的影响

摘　　要：目的探讨超短波疗法结合股四头肌训练对膝骨关节炎合并滑膜炎疗效及视觉模拟评分法(visual analogue score,VAS)、Ly-sholm膝关节评分量表(lysholm knee scoring scale,LKSS)、西安大略和麦克马斯特大学骨关节炎指数(western ontario and mcmaster university,WOMAC)的影响。方法选取2021年11月至2023年12月郑州大学第一附属医院收治的膝骨关节炎合并滑膜炎患者200例,按治疗方法不同将采取超短波疗法治疗的100例作为对照组,采取超短波疗法结合股四头肌训练的100例作为观察组,并对比两组的临床效果、视觉模拟疼痛(VAS)评分、膝关节LKSS评分、WOMAC评分及肌骨超声检查数据。结果观察组总有效率92.00%,高于对照组72.00%,差异有统计学意义(P<0.05);观察组治疗后VAS评分(4.30±0.95)分低于对照组(5.99±0.96)分,膝关节Lysholm评分(84.44±8.51)分高于对照组的(70.39±8.94)分,差异有统计学意义(P<0.05);观察组治疗后下蹲屈膝、正常活动、僵硬、上下楼梯、行走痛、久坐痛评分分别为(2.75±1.12)分、(2.66±0.97)分、(2.81±1.06)分、(2.56±1.03)分、(2.49±1.01)分及(2.75±1.13)分,均低于对照组相应评分,差异有统计学意义(P<0.05);观察组治疗后关节软骨厚度、滑膜厚度、关节积液范围分别是(2.79±0.30)mm、(1.52±0.44)mm及(2.27±0.66)cm2,改善情况均优于对照组,差异有统计学意义(P<0.05)。结论超短波疗法结合股四头肌训练对治疗膝骨关节炎合并滑膜炎具有显著效果,可有效缓解疼痛,提升膝关节功能,还可促进康复,值得推广、实施。Objective To investigate the effect of ultrashort wave therapy combined with quadriceps muscle training on knee osteoarthritis combined with synovitis and the effects of VAS,Lysholm and WOMAC scores.Methods A total of 200 patients with knee osteoarthritis combined with synovitis admitted to the First Affiliated Hospital of Zhengzhou University from November 2021 to December 2023 were selected and divided into the observation group and the control group,according to treatment method.The control group was treated with ultrashort wave therapy,and the observation group was treated with ultrashort wave therapy combined with quadriceps muscle training.The clinical effect,visual analogue pain(VAS)score,knee LKSS score,WOMAC score and musculoskeletal ultrasound data of the two groups were compared.Results The group observation total effective rate was 92.00%,higher than group control 72.00%(P<0.05).VAS and Lysholm scores of the two groups were compared before treatment(P>0.05).VAS scores of the observation group(4.30±0.95)after treatment were lower than that of the control group(5.99±0.96).Lysholm score of knee joint(84.44±8.51)was higher than that of control group(70.39±8.94)(P<0.05).After treatment,the scores of sitting pain,walking pain,walking up and down stairs,stiffness,normal activity,squatting and kneeling were(2.75±1.12)points,(2.66±0.97)points,(2.81±1.06)points,(2.56±1.03)points,(2.49±1.01)points and(2.75±1.13)points,respectively.The scores were lower than those of control group(P<0.05).The articular cartilage thickness,synovial thickness and articular volume of the observation group after treatment were(2.79±0.30)mm,(1.52±0.44)mm and(2.27±0.66)cm2,respectively,better than control group(P<0.05).Conclusion Ultrashort wave therapy combined with quadriceps muscle training has a significant effect on the treatment of knee osteoarthritis combined with synovitis,which can effectively relieve pain,improve the function of the knee joint,and promote rehabilitation.It is worth promoting and implementing.
