摘  要:目的本研究通过监测缺血伴非阻塞性冠状动脉疾病(ischemia with non-obstructive coronary arteries,INOCA)患者经过增强型体外反搏(enhanced external counterpulsation,EECP)治疗前后的生物学及功能学多项指标的变化,探究EECP对这一特殊群体的治疗效果。方法选取2023年1月至2023年12月在河南省直属机关第一门诊部与河南省胸科医院住院,符合INOCA诊断标准的患者80例,随机分为常规治疗组40例与联合治疗组40例。常规治疗组患者服用常规药物治疗,联合治疗组患者在给予常规药物基础上,额外增加一个疗程的EECP治疗。在治疗前后,对比两组血清一氧化氮(nitric Oxide,NO)、内皮素-1(endothelin-1,ET-1)、血管内皮生长因子(vascular endothelial growth factor,VEGF)和左心室射血分数(left ventricular ejection fraction,LVEF)、6分钟步行试验距离(6 minute walk distance,6MWD)、肱踝脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV)、西雅图心绞痛量表(seattle angina questionnaire,SAQ)等指标的变化。结果两组治疗后血清一氧化氮较治疗前升高、血清内皮素1降低、血清VEGF降低,但联合治疗组方案效果变化幅度更明显。两组治疗后的左心室射血分数改善、6MWD延长、SAQ分值提高、baPWV下降,但联合治疗组方案效果变化幅度更明显。结论EECP能够改善INOCA患者机体的生物标志物及功能性指标。Objective This study investigates the therapeutic effects of enhanced external counterpulsation(EECP)on patients with ischemia with non-obstructive coronary arteries(INOCA)by monitoring changes in various biological and functional parameters before and after treatment.Methods A total of 80 patients who met the INOCA diagnostic criteria and were hospitalized at the First Outpatient Department of Henan Province Directly Affiliated Organs and Henan Provincial Chest Hospital from January 2023 to December 2023 were selected and randomlydivided into the conventional treatment group of 40 cases and the combined treatment groupof 40 cases.Patients in the standard treatment group received conventional pharmacotherapy,while those in the combination treatment group were additionally administered a course of EECP therapy on top of the standard medication regimen.Changes in serum nitric oxide(NO),endothelin-1(ET-1),vascular endothelial growth factor(VEGF)and left ventricular ejection fraction(LVEF),6-minute walk distance(6MWD),brachial-ankle pulse wave velocity(baPWV),seattle angina questionnaire(SAQ)were compared between the two groups before and after treatment.Results After treatment,both groups exhibited an increase in serum nitric oxide levels,a decrease in serum endothelin-1,and a reduction in serum VEGF,with the combination treatment group showing more pronounced changes.Both groups also demonstrated improvements in left ventricular ejection fraction,extended 6MWD,increased SAQ scores,and reduced baPWV following treatment,with the combination treatment group again displaying more significant changes.Conclusion EECP has the potential to improve biomarkers and functional parameters in patients with INOCA.
