摘　　要：目的:探讨老年股骨转子间骨折患者采用侧卧位无牵引床股骨近端髓内钉内固定术的效果。方法:选取于六盘水第二人民医院骨科2021年1月—2024年5月行无牵引床股骨近端髓内钉内固定术治疗的老年股骨转子间骨折患者300例为研究对象,随机分为参照组与研究组,各150例。参照组术中取仰卧位,研究组术中取侧卧位。对比两组骨折复位效果及髋关节功能。结果:研究组侧位成角小于参照组(P<0.001);两组颈干角、骨折端侧位重叠率、骨折端前后位重叠率对比,无统计学差异(P>0.05)。术前,两组Harris髋关节评分系统评分对比,无统计学差异(P>0.05);术后6个月,两组Harris髋关节评分系统评分升高,研究组高于参照组(P<0.001)。结论:老年股骨转子间骨折患者采用侧卧位无牵引床股骨近端髓内钉内固定术的效果优于仰卧位,可缩小侧位成角,改善髋关节功能。Objective:To investigate the effect of proximal femoral intramedullary nailing without traction bed in lateral decubitus position for elderly patients with intertrochanteric fractures.Methods:A total of 300 elderly patients with intertrochanteric fractures treated by proximal femoral intramedullary nailing without traction bed in the Department of Orthopedics,Liupanshui Second People's Hospital from January 2021 to May 2024 were selected as the study subjects.They were randomly divided into a control group and a study group,with 150 cases in each group.The control group was placed in the supine position during surgery,while the study group was placed in the lateral decubitus position.The fracture reduction effect and hip joint function were compared between the two groups.Results:The lateral angulation in the study group was less than that in the control group(P<0.001).There was no significant difference between the two groups in the neck-shaft angle,lateral overlap of the fracture end,or anteroposterior overlap of the fracture end(P>0.05).There was no significant difference in the Harris Hip Score between the two groups before surgery(P>0.05).At 6 months postoperatively,the Harris Hip Scores of both groups increased,with the study group scoring higher than the control group(P<0.001).Conclusion:The effect of proximal femoral intramedullary nailing without traction bed in the lateral decubitus position is superior to that in the supine position for elderly patients with intertrochanteric fractures,as it reduces lateral angulation and improves hip joint function.
