摘 要:目的:分析2型糖尿病(T2DM)合并感染患者的感染情况及相关危险因素。方法:随机选取2022年10月—2023年10月克州人民医院内分泌科收治的100例T2DM患者开展研究。收集患者病历资料,根据患者是否合并感染分为感染组(n=42)与未感染组(n=58)。分析患者感染情况,并分析T2MD患者合并感染的危险因素。结果:T2DM合并感染患者以呼吸系统感染为主,占比40.48%(17/42);共分离病原菌72株,其中以革兰阴性菌为主,占比50.00%(36/72)。感染组女性、年龄≥60岁、病程≥10年、合并慢性阻塞性肺疾病、合并高脂血症、有吸烟史、有酮症酸中毒史、糖化血红蛋白(HbA1c)≥8%、空腹血糖(FBG)≥11 mmol/L、餐后2 h血糖≥16.7 mmol/L占比高于未感染组(P<0.05)。Logistic多因素分析显示,女性、HbA1c≥8%、FBG≥11 mmol/L是T2DM患者合并感染的独立危险因素(P<0.05)。结论:克州人民医院内分泌科T2DM合并感染患者呼吸系统感染为主,病原菌主要为革兰阴性菌,女性及HbA1c、FBG水平过高为T2DM合并感染的独立危险因素,临床应对高危患者加强预防及管理。Objective:To analyze the infection status and related risk factors in patients with type 2 diabetes mellitus(T2DM)complicated by infection.Methods:A total of 100 T2DM patients admitted to the Department of Endocrinology of Kezhou People's Hospital from October 2022 to October 2023 were randomly selected for the study.Patient case data were collected and divided into an infection group(n=42)and a non-infection group(n=58)based on the presence of infection.The infection status of patients was analyzed,and risk factors for infection in T2DM patients were identified.Results:Respiratory infections were the most common in T2DM patients with infection,accounting for 40.48%(17/42).A total of 72 pathogenic strains were isolated,with Gram-negative bacteria predominating at 50.00%(36/72).The infection group had a higher proportion of females,patients aged≥60 years,disease duration≥10 years,those with chronic obstructive pulmonary disease,hyperlipidemia,smoking history,history of ketoacidosis,glycosylated hemoglobin(HbA_(1c))≥8%,fasting blood glucose(FBG)≥11 mmol/L,and 2-hour postprandial blood glucose≥16.7 mmol/L compared to the non-infection group(P<0.05).Logistic multivariate analysis revealed that female gender,HbA_(1c)≥8%,and FBG≥11 mmol/L were independent risk factors for infection in T2DM patients(P<0.05).Conclusion:Respiratory infections are the primary infections in T2DM patients complicated by infection in the Department of Endocrinology of Kezhou People's Hospital,with Gram-negative bacteria being the predominant pathogens.Female gender and elevated HbA_(1c) and FBG levels are independent risk factors for infection in T2DM patients.Clinical prevention and management should be strengthened for high-risk patients.
