摘　　要：目的:探讨结核分枝杆菌复合群(MTBC)核酸检测联合结核分枝杆菌γ干扰素释放试验(TB-IGRA)在肺结核诊断中的应用效果。方法:选取2022年6月—2023年5月惠州市职业病防治院收治的120例疑似肺结核患者进行研究,采集所有研究对象痰液样本及血液样本,实施MTBC检测及TB-IGRA检测。以结核分枝杆菌培养结果为“金标准”,分析MTBC、TB-IGRA单独与联合诊断肺结核的效能(灵敏度、特异度、准确度)。结果:结核分枝杆菌培养结果显示,120例受试者确诊肺结核60例(50.0%)。联合检测诊断肺结核的灵敏度、准确度高于MTBC检测、TB-IGRA检测,差异有统计学意义(P<0.05);MTBC检测、TB-IGRA检测、两者联合检测诊断肺结核的特异度比较,差异无统计学意义(P>0.05)。结论:MTBC检测、TB-IGRA检测诊断肺结核的效果较好,可作为辅助诊断肺结核的方法,且联合检测能够提升诊断灵敏度及准确度。Objective:To explore the application effect of Mycobacterium tuberculosis complex(MTBC)nucleic acid detection combined with Mycobacterium tuberculosisγ-interferon release assay(TB-IGRA)in the diagnosis of pulmonary tuberculosis.Methods:From June 2022 to May 2023,120 patients with suspected pulmonary tuberculosis who were admitted to Huizhou Occu⁃pational Health Hospital were selected for the study.Sputum samples and blood samples of all subjects were taken.MTBC and TBIGRA tests were performed.Mycobacterium tuberculosis culture results were used as the"gold standard"to analyze the diagnostic efficacy(sensitivity,specificity,accuracy)of MTBC and TB-IGRA alone and in combination for pulmonary tuberculosis.Results:The culture results of Mycobacterium tuberculosis showed that 60(50.0%)of 120 subjects had a confirmed diagnosis of pulmonary tuberculosis.The sensitivity and accuracy of the combined test in the diagnosis of pulmonary tuberculosis were higher than those of MTBC test and TB-IGRA test,and the difference was statistically significant(P<0.05).There was no statistically significant differ⁃ence in the specificity of MTBC test,TB-IGRA test and combined test in the diagnosis of pulmonary tuberculosis(P>0.05).Conclu⁃sion:MTBC and TB-IGRA have better effect in the diagnosis of pulmonary tuberculosis,and can be used as auxiliary methods for the diagnosis of pulmonary tuberculosis,and the combined test can improve the sensitivity and accuracy of diagnosis.
