家庭关怀度在2型糖尿病患者健康信念与服药依从性间的中介作用分析

摘　　要：目的:分析家庭关怀度在2型糖尿病患者健康信念与服药依从性间的中介作用。方法:选取80例2型糖尿病患者作为观察对象,均为2022年2—8月徐州市第一人民医院收治,采用问卷调查的形式评估患者家庭关怀度、健康信念、服药依从性,分析2型糖尿病患者家庭关怀度、健康信念及服药依从性评分的相关性;构建结构方程模型,分析家庭关怀度在2型糖尿病患者健康信念与服药依从性间的中介作用。结果:80例2型糖尿病患者家庭关怀度评分为(5.38±1.12)分,条目均分为(0.54±0.14)分;健康信念评分为(96.35±15.22)分,条目均分为(2.75±0.62)分;服药依从性评分为(2.05±0.76)分,条目均分为(0.51±0.15)分。家庭关怀度评分与健康信念、服药依从性评分呈正相关关系(r=0.434、0.337,P=0.002、0.012),健康信念评分与服药依从性评分呈正相关关系(r=0.448,P=0.006)。2型糖尿病患者健康信念对其服药依从性的直接效应为0.422,家庭关怀度在健康信念、服药依从性间存在中介效应,间接效应为0.109,总效应为0.531(P<0.001)。结论:2型糖尿病患者服药依从性处于中等水平,与家庭关怀度及健康信念间存在相关关系,2型糖尿病患者的家庭关怀度在健康信念与药物依从性间存在中介作用。Objective:To analyze the mediating effect of family care on health beliefs and medication adherence in patients with type 2 diabetes mellitus.Methods:A total of 80 patients with type 2 diabetes mellitus were selected as observation subjects,all of whom were admitted to Xuzhou No.1 People's Hospital from February to August 2022.The family care,health belief and medica⁃tion adherence of patients were evaluated using questionnaires.The correlation between family care,health belief and medication adherence scores of patients with type 2 diabetes mellitus was analyzed.A structural equation model was constructed to analyze the mediating effect of family care on health belief and medication adherence of patients with type 2 diabetes mellitus.Results:The family care score in 80 patients with type 2 diabetes mellitus was(5.38±1.12),and the average score of items was(0.54±0.14).The health belief score was(96.35±15.22),and the average score of items was(2.75±0.62).The medication adherence score was(2.05±0.76),and the average score of items was(0.51±0.15).The family care score was positively correlated with health belief and medi⁃cation adherence scores(r=0.434,0.337,P=0.002,0.012),and health belief score was positively correlated with medication adher⁃ence score(r=0.448,P=0.006).The direct effect of health belief on medication adherence in patients with type 2 diabetes mellitus was 0.422.There was a mediating effect of family care on health belief and medication adherence,the indirect effect was 0.109,and the total effect was 0.531(P<0.001).Conclusion:The medication adherence of patients with type 2 diabetes mellitus is at an intermediate level,and has a correlation with family care and health belief.The health belief of patients with type 2 diabetes melli⁃tus can directly affect medication adherence,and can also affect medication adherence through the mediating effect of family care.
