摘　　要：目的:探讨气道分级管理联合早期分阶段康复锻炼在重症肺炎机械通气患者中的应用效果。方法:选取2023年1—12月于中国人民解放军陆军第80集团军医院行机械通气的重症肺炎患者84例作为研究对象,用随机数字表法分为对照组和观察组,各42例。两组均给予气道分级管理,对照组采用常规康复训练,观察组采用早期分阶段康复训练。比较两组恢复情况、肺功能指标及不良事件发生情况。结果:观察组机械通气时间、入住ICU时间、住院时间均短于对照组(P<0.05)。干预后,两组第1秒用力呼气容积、呼气流量峰值均高于干预前,且观察组高于对照组(P<0.05)。观察组不良事件总发生率低于对照组(P=0.024)。结论:气道分级管理联合早期分阶段康复锻炼在重症肺炎机械通气患者中的应用效果较好,能够缩短通气治疗时间、住院时间,改善患者的肺功能,减少不良事件。Objective:To explore the application effect of airway graded management combined with early staged rehabilitation exercise in mechanically ventilated patients with severe pneumonia.Methods:Eighty-four patients with severe pneumonia who underwent mechanical ventilation at the 80th Army Group Hospital of the PLA from January 2023 to December 2023 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 42 cases in each group.The control group adopted routine rehabilitation exercise,while the observation group adopted early staged rehabilitation exercise.The recovery,pulmonary function indexes and adverse events of the two groups were compared.Results:The duration of mechanical ventilation,length of ICU stay and length of hospital stay in the observation group were shorter than those in the control group(P<0.05).After intervention,the forced expiratory volume in 1 second and peak expiratory flow in the two groups were higher than those before the intervention,and these parameters in the observation group were higher than those in the control group(P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group(P=0.024).Conclusion:The application effect of airway graded management combined with early staged rehabilitation exercise in mechanically ventilated patients with severe pneumonia is good,which can shorten the duration of ventilation treatment and the length of hospital stay,improve lung function and reduce adverse events.
