黄连素对多囊卵巢综合征大鼠高雄激素状态及性激素结合球蛋白的影响

摘　　要：目的研究黄连素对多囊卵巢综合征(PCOS)大鼠治疗作用及机制。方法将48只6周龄SPF级雌性SD大鼠随机分为空白对照组(11只)和模型组(37只),模型组采用来曲唑[1 mg/(kg·d)]灌胃诱导PCOS模型。造模结束后,选取33只造模成功的大鼠随机分为模型对照组、二甲双胍组、黄连素组,每组11只。治疗期间空白对照组、模型对照组均用1%羧甲基纤维素灌胃,二甲双胍组用1%羧甲基纤维素+二甲双胍[135 mg/(kg·d)]灌胃,黄连素组用1%羧甲基纤维素+黄连素[81 mg/(kg·d)]灌胃。测定大鼠血清睾酮、空腹胰岛素,光镜下观察大鼠卵巢HE染色病理片和StAR免疫组化病理片。结果本实验模型大鼠出现典型的PCOS表现:卵巢有多囊改变;血清睾酮升高(P<0.05);空腹时胰岛素增加(P<0.01),表明存在胰岛素抵抗。二甲双胍和黄连素均能提高模型大鼠的胰岛素抵抗,不过黄连素在降低睾酮方面比二甲双胍效果好(P<0.05)。游离睾酮指数随着睾酮值的升高而升高。卵泡膜细胞的细胞膜上StAR密度会在睾酮值的升高而升高。结论黄连素不仅有治疗PCOS的作用,也能改善胰岛素抵抗、通过降低卵泡膜细胞的细胞膜上StAR密度降低血清睾酮值,治疗高雄激素血症。Objective To study the therapeutic effect and mechanism of berberine on polycystic ovary syndrome(PCOS)in rats.Methods A total of 486-week-old SPF female SD rats were randomly divided into blank control group(11 mice)and model group(37 mice).The model group was treated with letrozole[1 mg/(kg·d)]to induce PCOS model.After the modeling,the rats were randomly divided into model control group,metformin group and berberine group,with 11 rats in each group.During treatment,the blank control group and model control group were given 1%carboxymethyl cellulose by gavage,the metformin group was given 1%carboxymethyl cellulose+metformin[135 mg/(kg·d)]by gavage,and the berberine group was given 1%carboxymethyl cellulose+berberine[81 mg/(kg·d)]by gavage.Serum testosterone and fasting insulin were measured.HE staining and StAR immunohistochemical pathology of ovary were observed under light microscope.Results The typical manifestations of PCOS in this experimental model are:polycystic changes in ovary.Serum testosterone was increased(P<0.05).Insulin increased during fasting(P<0.01),indicating insulin resistance.Both metformin and berberine can improve insulin resistance in model rats,but berberine is better than metformin in reducing testosterone(P<0.05).The free testosterone index increased with the increase of testosterone value.The StAR density of the membrane of follicular membrane cells will increase with the increase of testosterone value.Conclusions Berberine not only has the effect of treating PCOS,but also can improve insulin resistance,reduce serum testosterone value by decreasing StAR density on the membrane of follicular membrane cells,and treat hyperandrogenemia.
