药师参与门诊对高血压患者用药、心理健康及血压控制水平的影响

摘　　要：目的评估药师在门诊管理中的介入对高血压患者的药物使用、心理健康状况及血压控制效果的影响。方法选取2024年1月至2024年6月在兴义市人民医院门诊就诊的高血压患者160例,以随机数字表法分为研究组(由药师参与提供个体化药物咨询、用药指导及心理支持,n=80)和对照组(常规医师管理,n=80)。统计患者入组3个月及入组6个月时的药学服务项目次数、心理状况、用药情况、血压控制情况及服药依从性。结果研究组在入组3个月和6个月时的药物重整、药物不良反应教育、生活方式教育的次数均高于对照组(均P<0.05);与入组当天比较,两组患者在入组3个月和6个月时的焦虑自评及抑郁自评评分均降低,且研究组更低(均P<0.05);研究组用药错误的总发生率低于对照组(P<0.05);在入组3个月和6个月时的收缩压和舒张压均低于入组当天,且研究组低于对照组(均P<0.05);研究组用药依从率高于对照组(P<0.05)。结论药师参与门诊管理能够改善高血压患者的用药准确性、心理健康状况、血压控制水平及服药依从性。Objective To evaluate the effects of pharmacists’intervention in outpatient management on drug use,mental health status and blood pressure control in hypertensive patients.Methods A total of 160 hypertensive patients treated in the outpatient department of Xingyi People’s Hospital from January 2024 to June 2024 were selected and divided into the study group(with pharmacists participating in providing individualized drug consultation,medication guidance and psychological support,n=80)and the control group(managed by conventional doctors,n=80)by random number table method.The number of pharmaceutical care items,psychological status,drug use,blood pressure control and medication compliance of patients at 3 months and 6 months of enrollment were counted.Results The frequency of drug modification,adverse drug reaction education and lifestyle education in the study group were higher than those in the control group at 3 and 6 months(all P<0.05).Compared with the enrollment day,the self-rated anxiety and depression scores were lower at 3 and 6 months in both groups,and lower in the study group(all P<0.05).The incidence of medication errors in the study group was lower than that in the control group(P<0.05).The systolic blood pressure and diastolic blood pressure were lower at 3 and 6 months,and the study group was lower than the control group(all P<0.05).The medication compliance rate of the study group was higher than that of the control group(P<0.05).Conclusions Pharmacists’participation in outpatient management can improve medication accuracy,mental health status,blood pressure control level and medication compliance of hypertension patients.
