瑞马唑仑与丙泊酚用于ICU紧急气管插管患者全身麻醉安全性和有效性分析

摘　　要：目的分析瑞马唑仑与丙泊酚用于ICU紧急气管插管全身麻醉的安全性和有效性。方法选择莆田市第一医院重症医学科2022年2月至2023年2月ICU收治的需进行气管插管全身麻醉的患者95例作为研究对象,以随机数字表法分为两组,分别是瑞马唑仑组(48例)和丙泊酚组(47例),对比两组患者用药后的起效、意识完全消失、气管插管时间,血流动力学变化,氧合指数,苏醒时间和不良反应发生率。结果两组患者的起效时间和气管插管时间差异无统计学意义(P>0.05),但瑞马唑仑组的意识完全消失时间短于丙泊酚组(P<0.05)。瑞马唑仑组麻醉后,平均动脉压、心率均低于丙泊酚组(均P<0.05)。瑞马唑仑组氧合指数高于丙泊酚组,苏醒时间短于丙泊酚组(P<0.05)。瑞马唑仑组的不良反应发生率低于丙泊酚组(P<0.05)。结论瑞马唑仑用于ICU紧急气管插管全身麻醉患者具有较高的安全性和有效性。与丙泊酚相比,瑞马唑仑起效时间更快,维持更稳定的血流动力学状态,缩短苏醒时间,并减少不良反应的发生。Objective To analyze the safety and efficacy of remifentanil and propofol for general anesthesia during emergency tracheal intubation in ICU.Methods A total of 95 patients requiring tracheal intubation general anesthesia admitted to the ICU of the Department of Intensive Care Medicine of Putian First Hospital from February 2022 to February 2023 were selected as the study objects,and were divided into two groups according to random number table method,namely remazolam group(48 cases)and propofol group(47 cases).The efficacy,complete loss of consciousness,intubation time,hemodynamic changes,oxygenation index,recovery time and incidence of adverse reactions were compared between the two groups.Results There was no significant difference in the onset time and tracheal intubation time between the two groups(P>0.05),but the time of complete loss of consciousness in remazolam group was shorter than that in propofol group(P<0.05).After anesthesia,the mean arterial pressure and heart rate in remazolam group were lower than those in propofol group(P<0.05).The oxygenation index of remazolam group was higher than that of propofol group,and the recovery time was shorter than that of propofol group(P<0.05).The incidence of adverse reactions in remazolam group was lower than that in propofol group(P<0.05).Conclusions Remazolam is safe and effective in ICU patients undergoing general anesthesia for emergency tracheal intubation.Compared with propofol,Remazolam takes effect faster,maintains a more stable hemodynamic state,shorens the time to recovery,and reduces the occurrence of adverse reactions.
