剖宫产子宫瘢痕妊娠诊断中经阴道彩超的应用价值

摘　　要：目的本研究旨在深入探究并评估经阴道彩超检查在剖宫产子宫瘢痕妊娠诊断中的临床应用价值与效果。方法本研究选取的时间范围为2022年1月至2023年12月,研究对象为泉州市妇幼保健院收治的剖宫产子宫瘢痕妊娠患者,共计64例。设计严格的纳入和排除标准,对所有入选患者均进行经阴道彩超检查和经腹部彩超检查。对比分析这两种检查方法的诊断结果,包括病灶的检出率、漏诊率、误诊率等指标,全面评估各自在剖宫产子宫瘢痕妊娠诊断中的效能。结果经阴道彩超能够更清晰地显示子宫切口瘢痕处的妊娠囊或混合回声包块,准确识别出病灶的形态特征。与此同时,与经腹部彩超相比,经阴道彩超的检出率更高,漏诊率更低(均P<0.05)。经阴道彩超和经腹部彩超的误诊率比较,差异无统计学意义(P>0.05)。结论相较于传统的经腹部彩超检查,经阴道彩超能够提供更清晰、更准确的影像学信息,提升诊断的准确率,降低漏诊和误诊的风险。Objective This study aims to explore and evaluate the vaginal colour to exceed examination in cesarean section uterine scar pregnancy diagnosis clinical application value and effect.Methods The time frame selected for this study was from January 2022 to December 2023,and the study subjects were patients with cesarean section uterine scar pregnancy admitted to Quanzhou Maternal and Child Health Hospital,totalling 64 cases.Strict inclusion and exclusion criteria were designed,and transvaginal ultrasound and transabdominal ultrasound were performed on all enrolled patients.The diagnostic results of these two examination methods were compared and analysed,including the detection rate of lesions,the leakage rate,the misdiagnosis rate and other indexes,to comprehensively evaluate the efficacy of each in the diagnosis of cesarean uterine scar pregnancy.Results Transvaginal ultrasound was able to show the gestational sac or mixed echogenic mass at the uterine incision scar more clearly,and accurately identify the morphological characteristics of the lesion.Meanwhile,compared with transabdominal ultrasound,transvaginal ultrasound had a higher detection rate and a lower missed diagnosis rate(both P<0.05).Comparison of misdiagnosis rates between transvaginal ultrasound and transabdominal ultrasound showed no statistically significant difference(P>0.05).Conclusions Compared with the traditional transabdominal color ultrasound examination,transvaginal color ultrasound can provide clearer and more accurate imaging information,improve the accuracy of diagnosis,and reduce the risk of missed diagnosis and misdiagnosis.
