摘 要:目的探讨营养学管理联合盆底康复量化活动对宫颈癌术后膀胱功能及盆底功能的影响。方法纳入2020年4月至2023年4月收治的宫颈癌患者90例为研究对象,随机分2组,每组45例。对照组采用常规干预措施,观察组采用营养学管理联合盆底康复量化活动进行干预,比较2组干预效果。结果术后残余尿量组间比较,观察组少于对照组(P<0.05);留置导尿管和自主排尿恢复时间组间比较,观察组短于对照组(P<0.05);膀胱颈移动度组间比较,观察组低于对照组(P<0.05)。2组干预后盆底障碍影响简易问卷-7(PFIQ-7)评分均低于干预前,观察组低于对照组(P<0.05);观察组干预后Oxford骨盆底肌力评分均高于干预前,观察组高于对照组(P<0.05)。2组干预后主观整体营养状况评估量表(PG-SGA)评分均低于干预前,观察组低于对照组(P<0.05)。2组干预后宫颈癌生活质量工具(QLICP-CE)评分均高于干预前(P<0.05);观察组QLICP-CE评分比对照组更高(P<0.05)。观察组术后并发症发生率为4.44%,低于对照组的20.00%(P<0.05)。结论宫颈癌手术患者中应用营养学管理联合盆底康复量化活动进行干预,可有效改善患者的术后膀胱功能、盆底功能和营养状况,提升其生活质量,减少术后并发症,具有较高的临床价值。Objective To investigate the effect of nutritional management combined with quantitative pelvic floor rehabilitation activities on bladder function and pelvic floor function after cervical carcinoma surgery.Methods A total of 90 cervical carcinoma patients admitted to our hospital from April 2020 to April 2023 were included as study subjects.They were randomly divided into the control group(n=45,conventional nursing)and observation group(n=45,nutritional management combined with pelvic floor rehabilitation quantitative activities).Intervention effects of the two groups were compared.Results Patients in the observation group had significantly less postoperative residual urine volume,shorter duration of indwelling catheter,faster recovery of automatic micturition,and lower bladder neck mobility than the control group(all P<0.05).The Pelvic Floor Impact Questionnaire—short form 7(PFIQ-7)and Patient-Generated Subjective Global Assessment(PG-SGA)scores were significantly reduced in both groups after intervention,which were significantly lower in the observation group than in the control group(all P<0.05).The Oxford Grading Scale score for pelvic floor,and the Quality of Life Instruments for Cancer Patients(QLICP-CE)score were significantly elevated in both groups after intervention,which were significantly higher in the observation group than in the control group(all P<0.05).The incidence of postoperative complications was significantly lower in the observation group than in the control group(4.44%vs 20.00%,P<0.05).Conclusion The application of nutritional management combined with quantitative pelvic floor rehabilitation activities for intervening surgically treated patients with cervical carcinoma can effectively improve the postoperative bladder function,pelvic floor function,nutritional status and quality of life,and reduce postoperative complications,which has high clinical value.
