摘　　要：目的探讨全方位干预对维持性血液透析(MHD)后血管钙化患者睡眠质量、生活质量的影响。方法将2022年1月至2023年12月收治的100例MHD后血管钙化患者作为研究对象,通过随机数字表法将其分为对照组(常规干预)和研究组(联合基于血管通路管理小组下全方位干预),每组50例。评价患者透析疗效、并发症率、心理状态、睡眠质量、生活质量。结果研究组人血蛋白指数高于对照组(P<0.05),MIS评分、并发症率低于对照组(P<0.05);研究组SAS、SDS评分低于对照组(P<0.05);研究组PSQI评分低于对照组(P<0.05);研究组SF-36评分高于对照组(P<0.05)。结论全方位干预在帮助MHD后血管钙化患者促进化疗效果提升,减少并发症发生方面效果突出,还可有效平复负性情绪,促进睡眠、生活质量提升,该模式具有较高临床应用价值。Objective To investigate the effects of holistic intervention by the vascular access management group on sleep quality and quality of life of patients with vascular calcification after maintenance hemodialysis(MHD).Methods A total of 100 patients with post-MHD vascular calcification admitted to our hospital from January 2022 to December 2023 were taken as the subjects of this study,and they were randomly divided into the control group(50 cases,conventional intervention)and the study group(50 cases,conventional intervention combined with holistic intervention by the vascular access-based management group)by the method of randomized numerical table.Dialysis efficacy,complication rate,psychological status,sleep quality,and quality of life were compared.Results The human blood protein index of the study group was significantly higher than that of the control group(P<0.05),while the Malnutrition-Inflammation Score(MIS)score and complication rate were significantly lower than those of the control group(P<0.05).The Self-Rating Anxiety Scale(SAS)and Self-Depression Anxiety Scale(SDS)scores of the study group were significantly lower than those of the control group(P<0.05).The Pittsburgh Sleep Quality Index(PSQI)of the study group was significantly lower than that of the control group(P<0.05),and the 36-Item Short Form Survey(SF-36)score of the study group was significantly higher than that of the control group(P<0.05).Conclusion The holistic intervention by the vascular access management group has outstanding effects on chemotherapy outcome,which can reduce the occurrence of complications,calm negative emotions and promote sleep and quality of life in post-MHD patients with vascular calcification.It shows a high clinical application value.
