丙种球蛋白联合糖皮质激素治疗儿童过敏性紫癜肾炎的临床疗效

摘　　要：目的探讨丙种球蛋白联合糖皮质激素治疗儿童过敏性紫癜性肾炎的临床疗效。方法选取2021年3月至2023年3月上饶市立医院收治的62例过敏性紫癜肾炎患儿作为研究对象,按照随机数字表法分为观察组与对照组,每组31例。对照组采取常规糖皮质激素治疗,观察组采取丙种球蛋白联合糖皮质激素治疗。比较两组治疗前后24 h尿红细胞、尿蛋白、免疫功能(CD4^(+)、CD8^(+))水平及临床疗效、不良反应发生情况。结果治疗后,两组24 h尿红细胞均少于治疗前,24 h尿蛋白水平均低于治疗前,且观察组24 h尿红细胞少于对照组,24 h尿蛋白水平低于对照组,差异有统计学意义(P<0.05)。治疗后,两组CD4^(+)水平均高于治疗前,CD8^(+)水平均低于治疗前,且观察组CD4^(+)水平高于对照组,CD8^(+)水平低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为96.77%,高于对照组的74.19%,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论丙种球蛋白联合糖皮质激素治疗过敏性紫癜肾炎患儿的疗效显著,能有效改善患儿尿红细胞与尿蛋白水平的异常变化,并增强机体免疫能力,且用药安全性较高,值得临床推广应用。Objective To investigate the clinical efficacy of gamma globulin combined with glucocorticoid in the treatment of hypersensitive purpura nephritis in children.Methods 62 children with hypersensitive purpura nephritis admitted to Shangrao Municipal Hospital from March 2021 to March 2023 were selected as the research subjects,they were divided into the observation group and the control group according to the random number table method,with 31 cases in each group.The control group was treated with conventional glucocorticoids,and the observation group was treated with gamma globulin combined with glucocorticoid.The levels of 24 h urine red blood cells,urine protein,immune function(CD4^(+),CD8^(+))before and after treatment,clinical efficacy and adverse reactions were compared between the two groups.Results After treatment,the 24 h urine red blood cells of the two groups were less than those before treatment,and the 24 h urine protein levels were lower than those before treatment,the 24 h urine red blood cells in the observation group were less than those in the control group,and the 24 h urine protein level was lower than that in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of CD4^(+)of the two groups were higher than those before treatment,the levels of CD8^(+)were lower than those before treatment,and the levels of CD4^(+)in the observation group were higher than those in the control group,the levels of CD8^(+)were lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 96.77%,which was higher than 74.19%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Gamma globulin combined with glucocorticoid is effective in the treatment of children with hypersensitive purpura nephritis,it can effectively improve the abnormal changes of
