摘　　要：目的探讨不同入路行椎弓根钉棒技术治疗腰椎骨折的临床疗效。方法选取2022年1—6月在抚州市第一人民医院接受手术治疗的60例脊柱骨折患者作为研究对象,根据不同入路方法分为经肌间隙入路组和经皮入路组,每组30例。经皮入路组采用微创经皮入路椎弓根钉棒手术,经肌间隙入路组采用经肌间隙入路椎弓根钉棒手术。比较两组手术相关指标、Cobb角、椎体前缘相对高度、视觉模拟评分法(visual analog scale,VAS)、Oswestry功能障碍指数(Oswestry disability index,ODI)及治疗优良率。结果两组术后下床时间、住院时间比较差异无统计学意义;经肌间隙入路组手术切口长度长于经皮入路组,手术时间短于经皮入路组,差异有统计学意义(P<0.05)。术后3个月,两组Cobb角均小于术前,椎体前缘相对高度均高于术前,差异有统计学意义(P<0.05)。术后3个月,两组VAS、ODI评分均低于术前,差异有统计学意义(P<0.05),两组间比较差异无统计学意义。两组治疗优良率比较差异无统计学意义。结论采用经肌间隙入路和微创经皮入路进行椎弓根钉棒手术治疗腰椎骨折的效果均较为理想,且经肌间隙入路操作更为简便。Objective To investigate the clinical efficacy of pedicle screw rod technique in the treatment of lumbar spine fractures by different approaches.Methods 60 patients with lumbar spine fractures who underwent surgical treatment in the First People's Hospital of Fuzhou from January to June 2022 were selected as the research subjects,they were divided into the transmuscular approach group and the percutaneous approach group according to different approaches,with 30 cases in each group.The percutaneous approach group was treated with minimally invasive percutaneous approach pedicle screw rod surgery,and the transmuscular approach group was treated with transmuscular approach pedicle screw rod surgery.The operation related indexes,Cobb angle,relative height of anterior vertebral body,visual analog scale(VAS),Oswestry disability index(ODI)and excellent and good rate of treatment were compared between the two groups.Results There were no significant differences in the time of getting out of bed and hospitalization between the two groups;the surgical incision in the transmuscular approach group was longer than that in the percutaneous approach group,and the operation time was shorter than that in the percutaneous approach group,the differences were statistically significant(P<0.05).At 3 months after operation,the Cobb angles of the two groups were smaller than that before operation,and the relative height of the anterior edge of the vertebral body was higher than that before operation,the differences were statistically significant(P<0.05),there was no significant difference between the two groups.At 3 months after operation,the VAS and ODI scores of the two groups were lower than those before operation,the differences were statistically significant(P<0.05).There was no significant difference in the excellent and good rate between the two groups.Conclusion The effect of pedicle screw rod surgery through transmuscular approach and minimally invasive percutaneous approach in the treatment of lumbar spine fracture is ideal,an
