摘 要:目的探讨小于孕龄与符合孕龄胎儿小脑发育情况及三维超声参数与孕龄的关系。方法选取2021年1月至2022年1月上饶市人民医院超声医学科60名建档孕妇作为研究对象,根据分娩孕龄分为早期早产组(31~33周)、晚期早产组(34~36周)、足月组(37~40周),每组20例,均行三维超声检查。比较小于孕龄与符合孕龄胎儿小脑发育三维超声特点及差异,并采用Pearson相关分析三维超声下小脑蚓部参数[蚓部前后径、蚓部上下径、蚓部周长、蚓部面积、脑干小脑蚓部(brainstem-vermis,BV)角、脑干小脑幕(brainstem-tentorium of cerebellum,BT)角]与孕龄的关系。结果小于孕龄胎儿小脑发育三维超声特点表现为小脑蚓部大小形态略正常,偶见双顶径、头围、头颅切面面积偏小,胎儿颅骨高回声环的延续性中断,颅内偶出现不对称局限性液性暗区;符合孕龄胎儿小脑发育三维超声特点表现为脑结构基本形成,颅骨光环呈椭圆形,脉络丛大小形状基本对称,类似于蝴蝶,矢状切面上可显示胼胝体周围动脉及其分支,透明隔腔位于前方,小脑位于后方;大脑冠状平面呈分开、对称的无回声卵形结构,经丘脑平面上显示丘脑呈低回声性的圆形对称结构;横切面成像还可见大动脉环。早期早产组中符合孕龄者与小于孕龄者蚓部前后径、蚓部上下径、蚓部周长、蚓部面积、BV角、BT角比较差异无统计学意义;晚期早产组与足月组符合孕龄者蚓部前后径、蚓部上下径、蚓部周长均长于小于孕龄者,蚓部面积、BT角均大于小于孕龄者,BV角小于小于孕龄者,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,符合孕龄者与小于孕龄者蚓部前后径、蚓部上下径、蚓部周长、蚓部面积、BT角与孕龄均呈正相关(r>0,P<0.05),BV角与孕龄呈负相关(r<0,P<0.05)。结论小于孕龄与符合孕龄胎儿小脑发育三维超声特点差异明显,可通过观察有无小�Objective To investigate the situation of cerebellar development in fetuses small and appropriate for gestational age,and the relationship between three-dimensional ultrasound parameters and gestational age.Methods 60 pregnant women who had established medical records in the Ultrasound Medicine department of Shangrao People's Hospital from January 2021 to January 2022 were selected as the research subjects,and they were divided into the early preterm birth group(31-33 weeks),the late preterm birth group(34-36 weeks)and the term group(37-40 weeks)according to the gestational age of delivery,with 20 cases in each group,all subjects received three-dimensional ultrasound examination.Three-dimensional ultrasound features of and differences in cerebellar development between fetuses with small and those with appropriate for gestational age,and Pearson correlation was used to analyze the relationship between three-dimensional ultrasound parameters of cerebellar vermis(anteroposterior diameter,suprainferior diameter,circumference,area,brainstem-vermis[BV]angle and brainstem-tentorium[BT]angle of fetal cerebellar vermis)and gestational age.Results The three-dimensional ultrasound characteristics of the cerebellum development of a fetus smaller than the gestational age were manifested as the size and shape of the vermis of the cerebellum being slightly normal,with occasional small biparietal diameter,head circumference,and head cross-sectional area,discontinuity of the continuity of the high-echo ring of the fetal skull,and occasional asymmetric localized liquid dark areas in the intracranial cavity;the three-dimensional ultrasound characteristics of the cerebellum development of a fetus in line with the gestational age were manifested as the basic formation of the brain structure,the skull halo being elliptical,the size and shape of the choroid plexus being basically symmetrical,similar to a butterfly,the corpus callosum and its branches could be displayed on the sagittal section,the septum pellucidum cavity was located in
