摘 要:目的探讨慢性丙型肝炎(chronic hepatitis C,CHC)患者中心外膜脂肪组织(epicardial adipose tissue,EAT)与胰岛素抵抗的相关性。方法选取2020年8月至2021年6月抚州市第一人民医院传染科收治的80例CHC患者作为研究对象,根据患者稳态模型评估胰岛素抵抗指数(homeostasis model assessment-insulin resistance index,HOMA-IR)分为非胰岛素抵抗组和胰岛素抵抗组,每组40例。比较两组胰岛素、生化指标[空腹血糖(fasting blood glucose,FBG)、甲胎蛋白(alpha-fetoprotein,AFP)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、C反应蛋白(C-reactive protein,CRP)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)]、体重指数(body mass index,BMI)、HOMA-IR、HCV病毒载量(HCV-RNA)、EAT厚度,分析EAT厚度与各项指标的相关性。结果两组FBG、AFP、TC、TG比较差异无统计学意义。胰岛素抵抗组EAT厚度厚于非胰岛素抵抗组,胰岛素、HOMA-IR、HCV-RNA、ALT、AST、AFP、CRP、LDL-C、BMI均高于非胰岛素抵抗组,HDL-C水平低于非胰岛素抵抗组,差异有统计学意义(P<0.05)。Pearson相关性析显示,EAT厚度与HOMA-IR、HCV-RNA、ALT、AST、CRP、BMI均呈正相关(r>0,P<0.05)。结论CHC患者EAT与胰岛素抵抗具有一定的相关性,可作为评估CHC患者发生胰岛素抵抗的辅助指标。Objective To investigate the correlation between epicardial adipose tissue(EAT)and insulin resistance in chronic hepatitis C patients(CHC).Methods A total of 80 patients with CHC admitted to the Infectious Disease department of First People's Hospital of Fuzhou from August 2020 to June 2021 were selected as the study subjects,the patients were divided into the non-insulin resistance group and the insulin resistance group according to homeostasis model assessment-insulin resistance index(HOMA-IR),with 40 cases in each group.The insulin,biochemical indexes(fasting blood glucose[FBG],alpha-fetoprotein[AFP],total cholesterol[TC],triglyceride[TG],alanine aminotransferase[ALT],aspartate aminotransferase[AST],C-reactive protein[CRP],low-density lipoprotein cholesterol[LDL-C],high-density lipoprotein cholesterol[HDL-C]),body mass index(BMI),HOMA-IR,HCV viral load(HCV-RNA)and EAT thickness were compared between the two groups,and the correlation between EAT thickness and each index was analyzed.Results There were no significant differences in FBG,AFP,TC and TG between the two groups;the thickness of EAT in the insulin resistance group was thicker than that in the non-insulin resistance group,the levels of insulin,HOMAIR,HCV-RNA,ALT,AST,AFP,CRP,LDL-C and BMI were higher than those in the non-insulin resistance group,and the level of HDL-C was lower than that in the non-insulin resistance group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed,EAT thickness was positively correlated with HOMA-IR,HCV-RNA,ALT,AST,CRP and BMI(r>0,P<0.05).Conclusions Epicardial adipose tissue has a certain correlation with insulin resistance in patients with CHC,which can be used as an auxiliary indicator to evaluate insulin resistance in patients with CHC.
